Il care management (CrM) si occupa del controllo del sistema e dell’erogazione dell’assistenza, coordina e organizza trasversalmente gli aiuti in campo sociosanitario, e ne rileva necessità strutturali. Nelle forme organizzative esistenti di case management (CsM) (gestione dimissioni, CsM assistenza domiciliare) mancano una prospettiva e competenze sovraordinate per trattare in modo equilibrato tanto l'aspetto umano quanto quello economico. Obiettivo del progetto CaRe è sviluppare, coordinare a livello regionale e valutare un servizio di CrM sovraordinato e interistituzionale, che funga da sportello di contatto e consulenza per la gestione delle cure e del sistema a beneficio degli anziani non autosufficienti e dei caregiver familiari. I vantaggi si manifestano sotto forma di sgravio per ospedali, ambulatori, servizio pubblico e sostenitori dei costi e portano, con la creazione di un innovativo servizio di CrM, a un cambiamento nell’area del programma a livello regionale verso una gestione e un coordinamento sostenibili della qualità delle cure e dell'efficienza dei costi. Della diffusione dei risultati si occupa una campagna di comunicazione transfrontaliera e la piattaforma web CaRe, che porta all'attenzione di un ampio pubblico le idee e i servizi del programma (curriculum formativo etc). Il valore aggiunto consiste nella creazione di una rete di operatori di cura locali e di una rete territoriale, nell'istituzione di un servizio di CrM e nella sua messa in funzione.

Care Management (CrM) beschäftigt sich mit der System- & Versorgungssteuerung, die fall- & einrichtungsübergreifend bedarfsgerechte Hilfen im Sozial- & Gesundheitswesen koordiniert, organisiert und strukturelle Voraussetzungen dafür aufzeigt. Diese übergeordnete Sichtweise & Kompetenz zur Ausbalancierung zw. Humanität & Ökonomie fehlt in existierenden Organisationsformen des Case Managements (CsM) (z.B. Entlassungsmanagement, CsM mobiler Pflegeanbieter). Ziel des CaRe-Projekts ist die Entwicklung, regionale Pilotierung & Evaluierung einer übergeordneten einrichtungsübergreifenden CrM-Stelle, die als regionale Anlauf- & Beratungsstelle zur System- & Versorgungssteuerung älterer pflegebedürftiger Menschen & deren pflegenden Angehörigen dient. Dieser Nutzen zeigt sich in der Entlastung für Krankenhäuser, ambulante Anbieter, öffentliche Seite & Kostenträger und führt durch Schaffung einer innovativen CrM-Stelle zur regionalen Veränderung im Programmgebiet im Sinne einer nachhaltigen Steuerung & Koordination der Pflegequalität & Kosteneffizienz. Für die Ergebnisverbreitung sorgt eine grenzüberschreitende Kommunikationskampagne, wie eine CaRe-Webplattform, die die daraus resultierenden Konzepte & Dienstleistungen (Ausbildungscurriculum, etc.) einer breiten Öffentlichkeit bekannt macht. Der Mehrwert liegt in der Vernetzung lokaler Versorgungsanbieter, Vernetzung innerhalb des Programmraumes & Schaffung einer CrM-Stelle und deren geplante Implementierung in den Regelbetrieb.

Description (EN):

Care Management (CrM) is concerned with the system & care control, which coordinates and organizes across cases and institutions needs oriented support in social- and health care and demonstrates structural prerequisites for it. This superordinate perspective & competence is missing in existing organizational forms of case management (CsM) (e.g. discharge management, CsM mobile care providers) in order to keep the balance between humanity and economics. The aim of the CaRe project is the development, regional piloting and evaluation of a superordinate crossinstitutional CrM office, which serves as a regional contact point & counseling center for the system & care management of elderly people in need of care and their caring relatives. The benefit is demonstrated in the relief to hospitals, outpatient providers, the public sector and funding agencies through the creation of an innovative CrM office that provides sustainable management & coordination of care quality & cost-efficiency. This leads to a regional change in the program area. A cross-border communication campaign provides result dissemination, such as a CaRe web platform, which makes the resulting concepts & services (training curriculum, a.s.o.) known to the public at large. The added value lies in the network of local care providers, networking within the program area & creation of a CrM office, and its planned implementation into standard operation.

Expected Results (IT):

Le esigenze individuali e domestiche delle persone non sono un fenomeno sociale nuovo. Le ritroviamo nei criteri di erogazione delle cure a livello ambulatoriale e ospedaliero. La loro portata diventa problematica con l'incremento delle cure ambulatoriali e la riduzione dei periodi di degenza ospedaliera a fronte del mutato scenario demografico. Anche con l'impiego delle forme organizzative di case management (O-CsM) già esistenti non si riesce a far fronte all'onere sempre più gravoso a carico degli interessati e del sistema, prodotto dalla diversità di amministrazioni, finanziamenti, organizzazioni a livello politico e geografico, dall'insufficienza della comunicazione e della cooperazione tra istituzioni, settori e professioni di cura. Non si riesce a coprire l'incremento di costi sanitari (5% annuo). Le opportunità risiedono nella creazione di sportelli di contatto e consulenza per gestire l'erogazione di cure e del sistema organizzativo a beneficio di anziani non autosufficienti e caregiver familiari. Così si riduce il rischio di dar vita a processi regionali di pianificazione sociosanitaria inefficienti, che si ripercuotono a lungo sulla qualità della cura e sull'efficienza dei costi. Per trovare equilibrio tra l'aspetto umanitario e quello economico nell'erogazione di cure a livello regionale, l'obiettivo è introdurre un modello di CrM per il coordinamento e la gestione di cure e consulenze e integrare le O-CsM. Per queste attività, sono stati sviluppati un modello transfrontaliero di best practice e un progetto pilota regionale con 3 elementi chiave: pianificazione sociale, sistema di cure sanitarie integrato, qualifiche personali e professionali. Grazie a interventi strategici integrati di cura e consulenza si ottiene un alleggerimento dell'onere umano ed economico a carico degli interessati e del sistema e grazie a una campagna di comunicazione transfrontaliera e a un programma di qualificazione si rafforzano i processi di pianificazione sociosanitaria.

Individuelle & häusliche Bedürfnisse der Menschen stellen kein neues soziales Phänomen dar. Sie finden sich im Versorgungsgrundsatz ambulant vor stationär wieder. Dessen Bedeutung zeigt sich besonders in der Problematik der zunehmenden ambulanten Versorgung durch den demografischen Wandel und die verkürzte Verweildauer im Krankenhaus. Auch der Einsatz bereits existierender Organisationsformen eines Case Managements (O-CsM) in den 3 Regionen kann daher die steigende Last auf der Betroffenen- & Systemebene aufgrund div. Verwaltungen, Finanzierungen, politische & geografische Ausrichtungen, mangelnde Kommunikation & Kooperation unter Versorgungseinrichtungen & -sektoren sowie Professionen nicht auffangen. Eine jährliche Kostensteigerung im Pflegebereich um 5% ist derzeit nicht mehr zu decken. Chancen liegen in der Schaffung einrichtungsübergreifender Anlauf- & Beratungsstellen zur System- und Versorgungssteuerung älterer pflegebedürftiger Personen und deren pflegenden Angehörigen. Damit soll das Risiko gesenkt werden, ineffiziente und ineffektive regionale Pflege- und Sozialplanungsprozesse zu gestalten, welche sich nachhaltig auf die Pflegequalität & Kosteneffizienz auswirken. Um regionale Versorgungsabläufe zwischen Humanität und Ökonomie auszubalancieren, ist das Ziel die Einführung eines Care Managementmodells (CrM) zur einrichtungsübergreifenden Steuerung & Koordination von Pflege & Betreuung sowie die Einbindung bestehender O-CsM. In den Aktivitäten wird ein grenzübergreifendes BPM und regionale Pilotierung mit dem Fokus auf 3 Kernelemente entwickelt: Sozialplanung, vernetztes pflegerisches Versorgungssystem, personelle & betriebliche Qualifizierung. Über vernetzte strategisch angelegte Pflege- & Betreuungsmaßnahmen wird die humanitäre & ökonomische Entlastung auf der Betroffenen- & Systemebene erreicht und über eine grenzüberschreitende Kommunikationskampagne & Qualifizierungsoffensive werden regionale Pflege- & Sozialplanungsprozesse gestärkt.

CO44 - Numero di partecipanti alle iniziative locali congiunte per l'occupazione e alle attività di formazione congiunta; Persone 30,00; Per gestire un’area di prova all’interno delle regioni del programma con un centro di care management per 12 mesi, ogni regione occuperà per almeno 1 persona. Verranno invitati circa 30 operatori di cura regionali a incontri di networking e di formazione per la realizzazione di un pilotaggio regionale. Tra questi: responsabili delle dimissioni, operatori di cura mobile, direttori di istituti di assistenza a lungo, breve termine e ciclo diurno, terapeuti, medici di base, rappresentanti di associazioni di volontariato, rappresentanti di casse malati ecc. OI7 - Numero di cooperazioni istituzionali nell’ambito della mobilità sostenibile; Numero 6,00; L’offensiva formativa comune a livello terziario richiede in base al processo di Bologna un’armonizzazione a livello europeo dei corsi e dei titoli di studio, tanto quanto una riforma universitaria transnazionale attraverso la mobilità internazionale degli studenti e dei docenti. Questo consiste nello sviluppo di un curriculum formativo comune che permetta a lungo termine di favorire una stretta cooperazione e la mobilità di personale docente, studenti e care manager (centri formativi, enti regionali). OI8 - Numero di cooperazioni istituzionali nell’ambito della sanità; Numero 75,00; Nelle rispettive aree di prova delle regioni del programma si instaurano almeno 25 cooperazioni con fornitori di servizi assistenziali. OI9 - Numero di nuovi concetti e servizi creati per migliorare la cooperazione tra amministrazioni e cittadini; Numero 12,00; Con il modello di best practice si concepisce un modello regionale sovraordinato che viene adattato di volta in volta alla rispettiva regione (= 4 concetti). Attraverso l’offensiva di qualificazione si sviluppa un curriculum formativo da adattare individualmente ad ogni regione (= 4 concetti). Il pilotaggio e la sua valutazione sono le premesse per il servizio di care management (=3 concetti / servizi). Per la sostenibilità ci si affida alla realizzazione di la piattaforma web CaRe a garanzia della trasparenza delle offerte per i cittadini (= 1 servizio / concetto).

CO44 - Zahl der Teilnehmer an gemeinsamen lokalen Beschäftigungsinitiativen oder Weiterbildungsmaßnahmen; Personen 30,00; Um einen Testbezirk innerhalb der Regionen des Programmraumes mit einer Care Managementstelle für 12 Monate abzudecken, wird pro Region mind. 1 Person angestellt. Etwa 30 Teilnehmer regionaler Versorgungsanbieter werden zu Vernetzungstreffen und Weiterbildungsmaßnahmen zur Umsetzung der regionalen Pilotierung eingeladen. Diese sind: Entlassungsmanager, Pflegepersonen in der mobilen Pflege und Betreuung, Leitungen von Langzeit-, Kurzzeit- & Tagespflegeeinrichtungen, Therapeuten, Hausärzte, Vertreter von Ehrenamtlichen Einrichtungen, Vertreter von Krankenkassen etc. Kooperationen im Bereich nachhaltige Mobilität; Anzahl 6,00; Die gemeinsame Qualifizierungsoffensive auf tertiärer Ebene fördert entsprechend des Bologna Prozesses eine europaweite Harmonisierung von Studiengängen und – abschlüssen sowie eine transnationale Hochschulreform durch die internationale Mobilität von Studierenden und Lehrenden. Dies besteht durch die Entwicklung eines gemeinsamen Ausbildungscurriculums, welches nachhaltig eine engere Kooperation und Mobilität von Lehrpersonal, Studenten und ausgebildeten Care Managern ermöglicht (Ausbildungszentren, Landeseinrichtungen). OI8 - Anzahl institutioneller Kooperationen im Gesundheitsbereich; Anzahl 75,00; In den jeweiligen Testbezirken der Regionen des Programmraumes werden mindestens 25 Kooperationen mit Versorgungsanbietern geschlossen. OI9 - Anzahl neu geschaffener Konzepte und Dienstleistungen zur Aufwertung der Zusammenarbeit von Verwaltungen und Bürger; Anzahl 12,00; Durch das Best Practice Modell wird ein übergeordnetes Modell konzeptioniert, welches auf die jeweilige Region adaptiert wird (= 4 Konzepte). Durch die Qualifizierungsoffensive wird ein überregionales Ausbildungscurriculum erstellt und an jede Region individuell angepasst (= 4 Konzepte). Als Voraussetzung für die Care Management Dienstleistung dient die Pilotierung und deren Evaluierung (= 3 Konzepte/Dienstleistungen). Im Sinne der Nachhaltigkeit dient die Dienstleistung der CaRe-Webplattform als Transparenz über die Angebote für die Bürger (= 1 Dienstleistung/Konzept).
