The treatment of patients will in the future be increasingly focused on 'personal regenerative medicine'. This is a new form of medicine in which doctors treat patients by creating new skin, bone or even an organ from cultured cells and biomaterials. This form of medicine generates great expectations in terms of innovative and personal care, but also has major challenges because it requires much high-quality knowledge and technology. Both Belgium and the Netherlands want to develop a knowledge economy to find answers to new challenges and at the same time to sustainably strengthen employment in the region. Platforms such as the Flemish RegMed and the Dutch Brightlands already support new developments within healthcare. To be able to test biomaterials on a large scale, a so-called 'organ-on-chip' offers new possibilities. These are small plates that mimic an organ or tissue and on which new biomaterials can be tested immediately in a physical environment. To develop this chip and prepare it for a broad introduction and use, a strong partnership is required consisting of both academic and industrial partners. 'Biomat' therefore creates a brand new microfluidic chip: the 'organ-on-chip 2.0'. This chip is seeded with stem cells to mimic a body's own organ or tissue, provided with supply and discharge of micro-fluid and sensitive sensors to perform measurements. The innovative aspect of this chip is the three-dimensional (3D) environment that is formed, in contrast to classic cell culture plates in which cells or tissues grow on a flat surface. Moreover, microfluidics - very small channels through which liquids are transported - mimic the transport of bodily fluids. To demonstrate the functionality of this revolutionary technology, examples will be constructed based on three types of clinical applications: bone, muscle and on-chip vascularized bone or muscle.

De behandeling van patiënten zal zich in de toekomst steeds meer richten op 'persoonlijke regeneratieve geneeskunde'. Dit is een nieuwe vorm van geneeskunde waarbinnen artsen patiënten behandelen door nieuwe huid, bot of zelfs een orgaan op te bouwen uit gekweekte cellen en biomaterialen. Deze vorm van geneeskunde wekt grote verwachtingen wat betreft innovatieve en persoonlijke zorg, maar kent ook grote uitdagingen omdat het veel hoogwaardige kennis en techniek vereist. Zowel België als Nederland willen hierin een kenniseconomie ontwikkelen. Platformen zoals het Vlaamse RegMed en het Nederlandse Brightlands ondersteunen reeds nieuwe ontwikkelingen binnen de gezondheidszorg. Om biomaterialen op grote schaal te kunnen testen, biedt een zogeheten 'orgaan-op-chip' nieuwe mogelijkheden. Dit zijn kleine plaatjes die een orgaan of weefsel nabootsen en waarop nieuwe biomaterialen direct kunnen worden getest in een lichaamsechte omgeving. Om deze chip te ontwikkelen en gereed te maken voor een brede introductie en gebruik, is een sterk partnerschap nodig van zowel academische als industriële partners. ‘Biomat’ creëert daarom een gloednieuwe microfluïdische chip: de 'orgaan-op-chip 2.0'. Deze chip wordt ingezaaid met stamcellen om een lichaamseigen orgaan of weefsel na te bootsen, voorzien van aan- en afvoer van micro-vloeistof en gevoelige sensoren om metingen te verrichten. Het vernieuwende aspect van deze chip is de 3D-omgeving die wordt gevormd, in tegenstelling tot klassieke celkweekplaten waarin cellen of weefsels op een vlak oppervlak groeien. Bovendien wordt door middel van microfluïdica - zeer kleine kanaaltjes waarin vloeistoffen worden vervoerd - het transport van lichaamsvloeistoffen nagebootst. Om de functionaliteit van deze revolutionaire technologie aan te tonen, zullen demonstrators worden gebouwd op basis van drie soorten klinische toepassingen: bot, spier en op-chip gevasculariseerde bot of spier. In de eerste twee jaar van het project zijn er belangrijke stappen gemaakt richting de ‘orgaan-op-chip 2.0’, door verschillende technische en biologische technologieën te ontwikkelen. Eerste ontwerpen en modellen van de chip zijn gemaakt. De ontwikkelde inzetstukken zijn geschikt gebleken voor het vormen van driedimensionale constructen van patiënt-eigen cellen en verschillende biomaterialen op microschaal. Daarnaast hebben we meerdere robuuste bioassays ontwikkeld voor het screenen van de toxiciteit/biocompatibiliteit en de functionaliteit van nieuw ontwikkelde biomaterialen. Voor het omhulsel van de chip, dat de constructen moet voorzien van voedingsstoffen, zijn ook verschillende ontwerpen getest, maar er wordt nog steeds gezocht naar de juiste combinatie van materialen en technieken om de benodigde resolutie te verkrijgen. Eerste succesvolle experimenten met impedantie sensoren, die de cellen en het vormen en verwijderen van weefsel kunnen meten, zijn uitgevoerd. De massa spectrometrische methode is verder gevalideerd om specifiek eluerende moleculen te kunnen bepalen op zeer laag niveau. Terwijl de chips getest zijn met cellijnen en primaire cellen van patiënten, is er ook veel werk gedaan aan het ontwikkelen van protocollen voor het differentiëren van humane geïnduceerde pluripotente stamcellen (hIPSCs) richting spierweefsel en botweefsel. Daarnaast zijn deze cellen voorzien van reporters waarmee hun groei en metabolisme niet-invasief gevolgd kunnen worden. Vorming van 3D microweefsels en organoiden met behulp van deze cellen is aangetoond. Ook is de ontwikkeling van het platform voor vascularisatie van microweefsels, Organo®Graft Plate vergevorderd. In de synthese en selectie van geschikte technieken om microschaal monsters van biomaterialen te maken, zijn belangrijke stappen gemaakt. Samenvattend, op alle aspecten van het project is voortgang geboekt. Integratie van verschillende componenten in de chip is ook gestart. De resultaten laten zien dat de iteratieve optimalisatie van verschillende onderdelen nodig is om demonstrators voor verschillende toepassingen te ontwikkelen en daarom zullen alle activiteiten in het project in verschillende mate gecontinueerd worden.

In addition, microfluidica — very small channels in which liquids are transported — simulates the transport of body fluids. To demonstrate the functionality of this revolutionary technology, demonstrators will be built based on three types of clinical applications: bone, muscle and on-chip vascularised bone or muscle. In the first two years of the project, important steps have been made towards the “organ-on-chip 2.0” by developing various technical and biological technologies. First designs and models of the chip were made. The developed inserts have proven to be suitable for forming three-dimensional constructs of patient-own cells and various microscale biomaterials. In addition, we have developed several robust bioassays to screen the toxicity/biocompatibility and functionality of newly developed biomaterials. For the shell of the chip, which must provide the constructs with nutrients, various designs have also been tested, but the right combination of materials and techniques is still being sought to obtain the required resolution. First successful experiments with impedance sensors, which can measure the cells and the formation and removal of tissue, have been carried out. The mass spectrometric method has been further validated to determine specifically eluating molecules at very low levels. While the chips have been tested with cell lines and primary cells of patients, much work has also been done to develop protocols for differentiating human-induced pluripotent stem cells (hiPSCs) towards muscle tissue and bone tissue. In addition, these cells are provided with reporters that allow their growth and metabolism to be monitored non-invasively. Formation of 3D microtissues and organoids using these cells has been demonstrated. The development of the platform for vascularisation of microtissues, Organo®Graft Plate, is also advanced. In the synthesis and selection of appropriate techniques to make microscale samples of biomaterials, important steps have been made. In summary, progress has been made on all aspects of the project. Integration of different components into the chip has also started. The results show that the iterative optimisation of different parts is needed to develop demonstrators for different applications and therefore all activities in the project will be continued to varying degrees.
