Description (EN):

The project creates web device and spots with medical files for the habitants of the area, where all of the data from the measurements will be uploaded and stored and made accessible at any time via the system by doctors volunteer,s etc., provides health and social care services to elderly inhabitants and disabled people, gives incentives to network participants, eliminates social discrimination and promotes equal treatment and social inclusion of people. Information added on 2021-02-10, regarding the mitigation of the effects of COVID-19 (the coronavirus pandemic that started in 2019) (English language only): The project’s main achievements include the upgrade of medical software that collects and records citizens’ medical data. The data is collected using a set of medical equipment (finger oximeters, digital thermometers, Nebulizers, Oxygen Cylinders with Flow meter and medical equipment measuring multiple parameters), which is available to citizens in need of medical care and measures multiple parameters (blood pressure, pulse, pulse regularity, temperature meter, SPO2 respiratory rate, 1–lead ECG). Four (4) regional medical offices have access to this data through the software, in order for regional doctors to update the citizens' medical records and provide them with their services. The offices’ staff, registers the citizens in the Health Software, completes their medical files and provides them with training on using the software/ equipment and on basic Covid-19 issues, like good nonmedical practices and protocols. Thus, the project, through the acquired equipment and the medical records, contributes to the response of Covid-19, since vital measurements such as oxygen level in blood and the respiratory level of the patient, are available to suggest if the patient has a medical problem or if s/he has contacted Covid-19.

Expected Results (EN):

Renovation and upgrade of existing social health care practices in the area, which will in turn increase the quality of the health care services provided to the habitants. The project will also result in the more immediate and effective response to emergency situations regarding patients that have certain limitations in transportation or physical abilities with Web Health - Social Care Responsive System and softwares which are going to be installed in the spots. The system will be able to store data and create a database for three municipalities in the CB area. This will provide the competent bodies with very significant statistical elements regarding the medical situation of the area. It will be easier to prevent emergency situations and be one step ahead of any imminent problems.
