Um auch in Zukunft eine qualitativ hochstehende Behandlung der Herzinsuffizienz (HI) zu gewährleisten, muss die medizinische Versorgung grundsätzlich verändert werden. HI ist eine der wichtigsten chronischen Erkrankungen; etwa 3.6M Menschen in NWE sind davon betroffen. In 2025 werden es >5M sein. Die Behandlung ist teuer (>3,600 €/Jahr/Patient) und wird ausschliesslich durch medizinisches Fachpersonal durchgeführt. Dies ist nicht aufrechtzuerhalten. Selbstbehandlung durch die Patienten könnte das Problem lösen, wird gegenwärtig aber nicht unterstützt. Aktuelle eHealth Produkte ersetzen die Behandlung nicht und sind lediglich zusätzlich zur normalen Behandlung. eHealth der nächsten Generation wird die Kosten der medizinischen Versorgung von HI durch aktiven Miteinbezug der Patienten signifikant reduzieren und zugleich die Behandlungsqualität verbessern. Dies besitzt ein enormes ökonomisches Potential. Die PASSION-HF Partner werden gemeinsam ein neues eHealth Produkt Abby genannt entwickeln, das HI Patienten erlaubt, sich zuhause selbständig zu behandeln. Es besitzt mehrere innovative Merkmale wie interaktives Interface mittels virtueller Pflegeperson, serious-gaming, neuartiges Feedback von Resultaten und selbstlernende Entscheidungshilfen für Patienten. PASSION-HF wird das ökonomische Potential nutzen, indem es das aktuelle TRL4 eHealth Produkt innerhalb von 43 Monaten auf TRL7 bringt. Dies führt zu wesentlichen Einnahmen im Gesundheitswesen. Das Resultat wird ein erfolgreich getestetes eHealth Produkt sein, das in 4 NWE Regionen implementiert ist. Schaffung von 10 Jobs im Gesundheitssektor, 4 unterstützte KMUs. Langfristigen Ziele nach 5 Jahren: Abby TRL 9; >20‘000 Patienten behandeln sich selbst, 70% der Behandlungsentscheide durch Abby, >16 neue KMUs, Kosten reduziert auf €1800/Jahr/Patient. 10 Jahre: >225‘000 Patienten behandeln sich selbst, 80% der Behandlungsentscheide durch Abby. > 30 neue KMUs. Kosten reduziert auf €1000/Jahr/Patient.

Description (FR):

Problème: changer le processus de soin est vital pour maintenir des soins de haute qualité dans le domaine de l’insuffisance cardiaque (IC). L’IC est une des plus importantes maladies chroniques, touchant ±3,600,000 pts dans NOE et >5,000,000 en 2025 en raison du vieillissement. Les soins de l’IC sont onéreux et uniquement dispensés par des professionnels de la santé. La charge de l’IC en soins et les coûts ne sont pas pérennes. Les auto-soins pourraient résoudre le problème mais ne sont pas soutenus dans la chaine de valeur. Les produits eHealth IC actuels ne remplacent pas les soins. Ils sont en supplément des soins standards plutôt que de soutenir l’autonomie du pt. Le eHealth nextgen proposera un soutien dans la décision, impliquera activement les pts IC et réduira les coûts (50%) tout en améliorant la qualité des soins. C’est un énorme potentiel commercial dans le domaine de la santé. Changement : Le consortium PASSION-IC va développer un produit eHealth intégré «Abby» permettant l’auto-soin des IC et l’auto-prescription. Il inclura des nouveaux paramètres tels qu’un soutien à la décision, un avatar interactif, des jeux pédagogiques, et un apprentissage autodidacte. PASSION-IC exploitera ce potentiel commercial en amenant le produit eHealth de son TRL4 à une application mobile personnalisée TRL7 en 43 mois. Ce qui permettra à un nombre croissant d’entreprises de commercialiser ces nouveaux produits de soins. Résultats: 4 PMEs commercialisant 1 nouveau produit eHealth IC en coopération avec des instituts de recherche, 10 emplois dans le secteur de la santé et mise en place d’un network international d’innovation. Effets long-terme : En 5 ans, Abby sera en TRL9, >20.000 pts en auto-soin dans NOE, 70% des décisions des traitements IC, 16 PME soutenant le processus des soins IC, coût approx.1800 €/an pp. En 10 ans, Abby opérationnel, >225.000 pts dans NOE, 80% des décisions des traitements, 30 PMEs soutenant le processus des soins IC, coûts app.1000€/an pp.

Issue: Changing the care process is vital to maintain high quality care in heart failure (HF). HF is one of the most prevalent chronic diseases, affecting 3,600,000 people in NWE today and >5,000,000 in 2025 due to population ageing. HF care is expensive (>3,600 €/y pp) and provided by healthcare professionals only. The burden of HF on healthcare labour and costs is unsustainable. Patient self-care may solve the problem but is unsupported in the value chain. Current HF-eHealth products do not replace care, are stand-alone and add-ons to standard care rather than supporting patients in self-care. Next-gen eHealth providing decision support and actively involving HF-patients will reduce costs (50%) and improve quality of care. This provides huge business potential for the healthcare value chain. Change: the PASSION-HF consortium will jointly develop an integrated eHealth product called “Abby” enabling self-care of chronic HF including self-prescription of medication. It will include novel features such as a decision support engine, interactive physician avatar interface, serious gaming tools and self-learning feedback system. PASSION-HF will exploit this business potential by bringing the current TRL4 eHealth product to a TRL7 personalized web-based application within 43 months. This leads to an increased number of enterprises supported to introduce new healthcare products. Outputs: 4 supported SMEs introducing 1 new to the market HF eHealth product in cooperation with research institutions, 10 jobs in the healthcare sector and the establishment of 1 new transnational innovation network. Long-term effects: By year 5, Abby at TRL 9, >20,000 patients in performing self-care in NWE with Abby, 70% treatment decisions by Abby, >16 SMEs supporting the treatment process, costs app. 1,800 €/y pp. By year 10, Abby operational, >225,000 patients in NWE, 80% treatment decisions by Abby, >30 SMEs supporting HF-treatment process, costs app. 1,000 €/y pp.
