The project describes and simulates care chains that span both primary care and specialist care as well as home care, and optimise these with regard to factors such as lead times, costs and quality of care. The purpose of the project is to develop routines and working methods to follow a patient’s path through the healthcare device and carry out impact assessments of changes in the patient’s care chain with support from available simulation tools. The goal is to describe and simulate care chains that span both primary care and specialist care as well as home care, and optimise these with regard to factors such as lead times, costs and quality of care. The focus will be on a few well-defined patient groups with a high level of care consumption.

Projektet beskriver och simulerar vårdkedjor som sträcker sig över såväl primärvård och specialistvård som hemsjukvård, och optimera dessa med avseende på faktorer som ledtider, kostnader och vårdkvalitet. Syftet med projektet är att utarbeta rutiner och arbetssätt för att följa en patients väg genom vårdapparaten och utföra konsekvensanalyser av förändringar i patientens vårdkedja med stöd av tillgängliga simuleringsverktyg. Målet är att beskriva och simulera vårdkedjor som sträcker sig över såväl primärvård och specialistvård som hemsjukvård, och optimera dessa med avseende på faktorer som ledtider, kostnader och vårdkvalitet. Fokus kommer att ligga på ett fåtal väl definierade patientgrupper med stor vårdkonsumtion.
