Risks related to the use of drugs increase with age and can have serious consequences (hospitalizations, deaths). These risks, referred to as drug-related iatrogenia, are largely preventable thanks to better collaboration between health professionals on the correct use of the drug. With this in mind, OPTIMAGE aims to deploy in the cross-border region of the Pyrenees a project for the prevention of drug-related iatrogenia in the elderly. Developed in connection with the APTITUDE project on the prevention of dependence among seniors, OPTIMAGE will be integrated into the APTITUDE network by strengthening it through the integration of city and hospital pharmacists. 3 axes are planned: - Creation of the network of fight against drug-related iatrogenia (RELIM) which will be based on the existing devices in the territories. It will involve pharmacists, family doctors, nurses, geriatricians, hospitals... Structures located in isolated geographic areas will be included as a priority. - Training / information for health professionals, the general public, associations, pharmaceutical companies. - Animation of the network with deployment of 2 flagship actions, one concerns seniors at home, and the other hospitalized seniors. This cross-border project should help to standardize practices, promote exchanges and strengthen actions in terms of prevention and reduction of drug-related iatrogenia among seniors.

Los riesgos asociados con el uso de fármacos aumentan con la edad y pueden tener consecuencias graves (hospitalizaciones, muertes). Estos riesgos, denominados iatrogenia medicamentosa, se pueden prevenir en gran medida gracias a una mejor colaboración entre los profesionales sanitarios sobre el uso del medicamento. Por eso, OPTIMAGE pretende desplegar un proyecto en la zona transfronteriza de los Pirineos para prevenir la iatrogenia medicamentosa (IM) en las personas mayores. Desarrollado en relación con el proyecto APTITUDE sobre la prevención de la dependencia entre las personas mayores, OPTIMAGE se agregará a la red APTITUDE fortaleciéndola. Se prevén 3 ejes: - Creación de la Red de Lucha contra la iatrogenia medicamentosa (RELIM), que se basará en los mecanismos existentes en los territorios. Participarán farmacéuticos, médicos de familia, enfermeras privadas, geriatras, hospitales... Se incluirán prioritariamente las estructuras situadas en zonas geográficas aisladas. - Formación/información de los profesionales sanitarios, del gran público, de las asociaciones, de las empresas farmacéuticas. - Animación de la red con 2 acciones emblemáticas, una dirigida a las personas mayores en el domicilio, y la otra a las personas mayores hospitalizadas. Este proyecto transfronterizo debe permitir homogeneizar las prácticas, promover los intercambios y reforzar las acciones de prevención y reducción de los IM en las personas mayores.

Les risques liés à l’utilisation des médicaments augmentent avec l’âge et peuvent avoir des conséquences sévères (hospitalisations, décès). Ces risques, désignés sous le terme d’iatrogénie médicamenteuse, sont en grande partie évitables notamment grâce à une meilleure collaboration entre professionnels de santé sur le bon usage du médicament. Fort de ce constat, OPTIMAGE vise à déployer dans la zone transfrontalière des Pyrénées un projet de prévention de l’iatrogénie médicamenteuse (IM) chez les personnes âgées (PA). Elaboré en lien avec le projet APTITUDE portant sur la prévention de la dépendance chez les PA, OPTIMAGE se greffera sur le réseau APTITUDE en le renforçant. 3 axes sont prévus : - Création du Réseau de Lutte contre l’Iatrogénie Médicamenteuse (RELIM) qui s’appuiera sur les dispositifs existants dans les territoires. Il associera les pharmaciens, les médecins de famille, les infirmiers libéraux, les gériatres, les établissements hospitaliers... Les structures situées dans les zones géographiques isolées seront inclues en priorité. - Formation/information des professionnels de santé, du grand public, des associations, des entreprises pharmaceutiques. - Animation du réseau avec 2 actions phares, l’une visant les PA à domicile, et l’autre les PA hospitalisées. Ce projet transfrontalier doit permettre d’homogénéiser les pratiques, de favoriser les échanges et de renforcer les actions en termes de prévention et réduction de l’IM chez la PA.
