Description (EN):

In continuity with the DialysIS project (Interreg 2007-2013), the project aims to improve the quality of life (QoL) of dialysis patients living on the Swiss/Italian border, reducing mortality rates and comorbidities. Various activities are proposed to address the problem both from a clinical and organizational point of view: 1. multicenter validation of decision support systems whose integration into clinical protocols, will allow to customise the dialysis prescription, improving the patient’s QoL; 2. the domiciliation of treatments, optimal solution in cross-border and mountain contexts; 3. a dashboard of indicators to monitor the performance of the centers; 4. the sharing of guidelines, protocols and best practices, to ensure territorial continuity of care, while ensuring the patient a personalised and high quality therapy.

Description (IT):

In continuità con il progetto DialysIS (Interreg 2007-2013), il progetto si propone di migliorare la qualità della vita (QoL) dei pazienti in dialisi che vivono sul confine Svizzero/Italiano, riducendo tassi di mortalità e comorbidità. Sono proposte diverse attività per affrontare il problema sia dal punto di vista clinico che organizzativo: 1. la validazione multicentrica di sistemi di supporto decisionale la cui integrazione nei protocolli clinici, permetterà di personalizzare la prescrizione dialitica, migliorando la QoL del paziente; 2. la domiciliazione dei trattamenti, soluzione ottimale in contesti transfrontalieri e montani; 3. un cruscotto di indicatori per monitorare le performance dei centri; 4. la condivisione di linee guida, protocolli e best practice, per garantire la continuità territoriale della cura, assicurando nel contempo al paziente una terapia personalizzata e di alta qualità.

Achievements (EN):

The project, created to improve the quality of life of dialysis patients, has created and shared an online platform to collect patients' clinical data and to encourage the mobility of those undergoing life-saving therapy. The project has identified targeted and personalized treatment paths to allow patients to benefit from homogeneous treatment, in centers different from the reference one, ensuring the patient a personalized and high-quality therapy. The platform, also useful for sharing protocols to reduce the side effects of dialysis treatment, has made the multi-center validation of decision support systems possible, the integration of which into clinical protocols has made it possible to personalize the therapeutic prescription. The possibility of home-based treatments was also studied, an optimal solution in cross-border and mountain contexts and to offer the patient the possibility of greater autonomy and freedom to move around the area for leisure or work, guaranteeing territorial continuity of treatment.

Expected Results (IT):

Interventi di integrazione e scambio di informazioni, tra i centri operanti in ambiti territoriali diversi, coordinamento delle attività di cura. Piattaforma web per l'interazione con le reti di patologia croniche esistenti e condividere buone pratiche, esperienze e protocolli clinici in ambito nefrologico per i medici e per i pazienti link e informazioni utili. Sperimentazione di iniziative per accrescere l'accessibilità e la qualità dei servizi sanitari in ambito dialitico. Valutazione costi/benefici di iniziative per accrescere l'accessibilità dei servizi sanitari in ambito dialitico. Ampliamento delle informazioni relative ad ogni specifico trattamento, permettendo di meglio e più prontamente caratterizzare il rischio di insorgenza di ipotensione intradialitica. Adozione di soluzioni tecnologiche innovative volte al miglioramento della qualità della cura dei pazienti in dialisi, nonché della prevenzione delle comorbidità conseguenti all’insorgenza ripetuta di IDH durante i trattamenti.

Piattaforma web Rrealizzazione di un sistema di indicatori per il monitoraggio delle prestazioni di reparto; Sviluppo sistema di reporting; Sperimentazione di misure di Patient Experience. Protocolli per l’automatizzazione della registrazione di dati dialitici Documento di analisi dei possibili percorsi di domiciliazione/ delocalizzazione del trattamento. Ottimizzazione di modelli compartimentali paziente- specifici. Valutazione di esportabilità ed applicabilità anche in altri centrI Integrazione tra modello ottimizzato ed algoritmi per l’identificazione dell’IDH .
