Digitization heralds the future of healthcare. This project focuses on digitizing pathology, which holds a crucial position in clinical workflows for diagnostics, yet there is a shortage across the region. Image sharing and analysis in combination with artificial intelligence (AI), termed “Digital Pathology” (DP), holds the potential for faster and even more accurate diagnosis. Developing AI-based solutions is challenging, requiring large volumes of high-quality data and interdisciplinary collaborations between pathologists and data scientists. Yet, this unique expertise is present in the Meuse-Rhine region. We aim to develop the first cross-border DP platform that enables pathologists from different regions to collaborate, facilitating a faster and more precise patient diagnosis. In the platform, we will implement existing AI-algorithms and new algorithms that will be developed in this project. These algorithms will aid in patient diagnosis and therapy response for lung cancer and colorectal diagnoses, improving healthcare in our region. We will also investigate a novel molecular imaging technique, mass spectrometry imaging, as the next-generation innovation in the DP field. This project will contribute to the adaptation of cutting-edge technologies in the region, facilitating the uptake of AI in hospitals, and advancing research and innovation regarding DP in the region, therefore strongly contributing to SO 1.i. This will lead to improved healthcare in the Euregion in two diseases with a high unmet need, leading to healthier citizens. By combining the strengths of our organizations in the Euregion, we expect to significantly impact the clinical use of DP, attract more talents to our region, and create opportunities for new SMEs to establish themselves in this innovative ecosystem. We will be the first to adopt a multi-cross-border, multilingual approach for the clinical use of DP, which will serve as a use case for other regions in Europe and beyond. Our consortium comprises several organizations with large cohorts of material, which can be shared via data agreements. Combined with strong knowledge in machine learning, we are uniquely positioned to develop clinical-grade algorithms that can benefit SMEs in the region. The two SMEs involved are experts in digital pathology software, while M4i is highly skilled in Mass spectrometry imaging. Therefore the complementary expertise from the partners in this project will ensure a cross-border partnership that will endure beyond this project. To ensure the project’s sustainability and future regional impact, the platform and generated (anonymized) data can be made available to hospitals and companies according to GDPR. Legal aspects regarding cross-border regulations will be examined, and we will set up a create a multi-stakeholder advisory board. Finally, we will develop an international training network on DP. To answer to these needs, and due to inflation, our budget has increased by 20% compared to step 1. The project will start on September 1st due to the summer break and need to hire staff.

Dit project richt zich op het digitaliseren van de pathologie, een cruciaal onderdeel van de klinische workflows, waar in de hele regio een tekort aan is. Het delen en analyseren van beelden in combinatie met kunstmatige intelligentie (AI), ook wel "Digitale Pathologie" (DP) genoemd, biedt de mogelijkheid voor een snellere en nog nauwkeurigere diagnose. Het ontwikkelen van op AI gebaseerde oplossingen is een uitdaging en vereist grote hoeveelheden gegevens van hoge kwaliteit en interdisciplinaire samenwerking tussen pathologen en datawetenschappers. Toch is deze unieke expertise aanwezig in de Maas-Rijn regio. We willen het eerste grensoverschrijdende DP-platform ontwikkelen dat pathologen uit verschillende regio's in staat stelt om samen te werken, waardoor een snellere en preciezere diagnose van patiënten mogelijk wordt. In het platform implementeren we bestaande en nieuwe AI-algoritmen die in dit project worden ontwikkeld. Deze algoritmen zullen helpen bij de diagnostiek en de therapierespons van patiënten voor longkanker en colorectale diagnoses, waardoor de gezondheidszorg in onze regio zal verbeteren. We zullen ook een nieuwe moleculaire beeldvormingstechniek onderzoeken, massaspectrometriebeeldvorming, als de volgende generatie innovatie op het gebied van DP. Dit project zal bijdragen aan de introductie van geavanceerde technologieën in de regio, het gebruik van AI in ziekenhuizen bevorderen en onderzoek en innovatie op het gebied van DP in de regio stimuleren, wat sterk bijdraagt aan doelstelling 1.i. Om de duurzaamheid van het project en de toekomstige regionale impact te garanderen, kunnen het platform en de gegenereerde (geanonimiseerde) gegevens beschikbaar worden gesteld aan ziekenhuizen en bedrijven volgens GDPR. Juridische aspecten omtrent grensoverschrijdende regelgeving zullen worden onderzocht en we zullen een multi-stakeholder adviesraad oprichten. Tot slot zullen we een internationaal trainingsnetwerk over DP ontwikkelen. Om aan deze behoeften te voldoen, en vanwege de inflatie, is ons budget met 20% verhoogd ten opzichte van stap 1. La numérisation des soins de santé est un enjeu majeur de nos sociétés. La pathologie occupe une place cruciale dans les flux de diagnostic en milieu hospitalier, mais il existe une pénurie d'expert. Ce projet se concentre sur la numérisation de la pathologie. Le partage et l’analyse d’images en combinaison avec l’intelligence artificielle (IA), appelée « pathologie numérique » ou "Digital Pathology" (DP), offrent le potentiel de diagnostics plus rapide et plus précis. Le développement de solutions basées sur l'IA est un défi, nécessitant de grands volumes de données de haute qualité et des collaborations interdisciplinaires entre pathologistes et data scientists. Ce savoir-faire unique est présent dans la région Meuse-Rhin. Dans l'objectif d'améliorer les soins de santé, nous visons à développer la première plateforme DP transfrontalière pour permettre aux pathologistes de différentes régions de collaborer et faciliter un diagnostic plus rapide et plus précis des pathologies. Dans la plateforme, nous implémenterons des algorithmes d'IA existants ainsi que de nouveaux algorithmes développés dans ce projet. Ces AI faciliteront le diagnostic des patients et la réponse thérapeutique pour les cancers du poumon et colorectal. Nous étudierons également une nouvelle technique d'imagerie moléculaire, l'imagerie par spectrométrie de masse, innovation de nouvelle génération dans le domaine de la DP. Ce projet contribuera à l'adaptation des technologies de pointe dans la région, en facilitant l'adoption de l'IA dans les hôpitaux et en faisant progresser la recherche et l'innovation en DP, contribuant ainsi fortement à l'SO 1.i. Cela conduira à une amélioration des soins de santé des citoyens dans l’Euregio pour deux maladies présentant d’importants besoins non satisfaits. En combinant les forces de nos organisations, nous espérons avoir un impact significatif sur l'utilisation clinique de la DP, attirer davantage de talents dans notre région et créer des opportunités pour l'établissement de nouvelles PME dans cet écosystème innovant. Nous serons les premiers à adopter une approche multilingue et transfrontalière de l’utilisation clinique de la DP, qui servira d'exemple pour d’autres régions. Notre consortium comprend plusieurs organisations disposant de grandes cohortes de matériel, qui peuvent être partagées via des accords de données. Combinés à de solides connaissances en apprentissage automatique, nous sommes dans une position unique pour développer des algorithmes de qualité clinique. Les deux PME impliquées sont expertes en logiciels de pathologie numérique, tandis que M4i est hautement compétente en imagerie par spectrométrie de masse. Par conséquent, l'expertise complémentaire des partenaires de ce projet garantira un partenariat transfrontalier qui perdurera au-delà de ce projet. Pour garantir la durabilité du projet et son impact régional futur, la plateforme et les données générées (anonymisées) peuvent être mises à la disposition des hôpitaux et des entreprises conformément au RGPD. Les aspects juridiques concernant la réglementation transfrontalière seront examinés et nous mettrons en place un comité consultatif multipartite. Enfin, nous développerons un réseau international de formation sur la DP. Pour répondre à ces besoins, notre budget a augmenté de 20% par rapport à l'étape 1 (principalement du à l'inflation). Le projet démarrera le 1/9/2024 en raison des vacances d'été et de la nécessité d'embaucher du personnel.

Dieses Projekt konzentriert sich auf die Digitalisierung der Pathologie, die in den klinischen Arbeitsabläufen für die Diagnostik eine entscheidende Rolle spielt, in der Maas-Rhein-Region jedoch ein Mangel besteht. Die gemeinsame Nutzung und Analyse von Bildern in Kombination mit künstlicher Intelligenz (KI), auch „Digitale Pathologie“ (DP) genannt, birgt das Potenzial diese Lücke zu schließen. Die Entwicklung KI-basierter Lösungen ist eine Herausforderung und erfordert große Mengen hochwertiger Daten und eine interdisziplinäre Zusammenarbeit zwischen Pathologen und Datenwissenschaftlern. Dieses einzigartige Fachwissen ist jedoch in der Maas-Rhein-Region vorhanden. Unser Ziel ist es, die erste grenzüberschreitende DP-Plattform zu entwickeln, die Pathologen aus verschiedenen Regionen die Zusammenarbeit ermöglicht und so eine schnellere und präzisere Patientendiagnose ermöglicht. In der Plattform werden wir sowohl bereits bestehende als auch von diesem Projekt neu entwickelte KI-Algorithmen implementieren. Wir werden auch eine neuartige molekulare Bildgebungstechnologie, die bildgebende Massenspektrometrie, als Innovation für die DP untersuchen. Dieses Projekt wird daher zur Anwendung modernster Technologien in der Region beitragen, die Einführung von KI in Krankenhäusern beschleunigen und Forschung und Innovation im Bereich DP in der Region vorantreiben. Damit wird ein wichtiger Beitrag zu SO 1.i geleistet. Dies wird zu einer verbesserten Gesundheitsversorgung der Bürger in der Euregio führen, mit Fokus auf das Therapieansprechen bei Lungenkrebs und Darmdiagnosen, beide mit hohem ungedecktem Bedarf. Wir erwarten, dass die Zusammenarbeit unserer Organisationen in der Euregio den klinischen Einsatz von DP erheblich fördern wird, mehr Fachkräfte in unsere Region locken und Möglichkeiten für neue KMU schaffen wird, sich in diesem innovativen Ökosystem zu etablieren. Wir werden die Ersten sein, die einen grenzüberschreitenden, mehrsprachigen Ansatz für den klinischen Einsatz von DP übernehmen, der als Blaupause für andere Regionen in Europa und darüber hinaus dienen wird. Unser Konsortium besteht aus mehreren Organisationen mit großen Materialkohorten, die über Vereinbarungen gemeinsam genutzt werden können. Ausgestattet mit fundierten Kenntnissen im maschinellen Lernen sind wir in der einzigartigen Position, Algorithmen auf klinischer Ebene zu entwickeln, von denen KMU in der Region profitieren können. Die beiden beteiligten KMU sind Experten für digitale Pathologiesoftware, während das M4i über umfassende Kenntnisse in der bildgebenden Massenspektrometrie verfügt. Daher wird das komplementäre Fachwissen dieses Projektes eine grenzüberschreitende Partnerschaft gewährleisten, die über dieses Projekt hinaus Bestand haben wird. Um eine nachhaltige Wirkung des Projekts auf die Region auch zukünftig sicherzustellen, werden die Plattform und die generierten (anonymisierten) Daten Krankenhäusern und Unternehmen gemäß DSGVO zur Verfügung gestellt. Wir werden rechtliche Aspekte grenzüberschreitender Regulierungen prüfen und einen Interessensvertreter-Beirat einrichten. Zudem werden wir ein internationales Trainingsnetzwerk zum Thema DP aufbauen. Um diesen Bedarf zu decken, wurde unser Budget aufgrund der Inflation im Vergleich zu Schritt 1 um 20% erhöht. Das Projekt wird aufgrund der Sommerpause und der Notwendigkeit, Personal einzustellen, am 1. September starten.

Actual Achievements (EN):

This project aims to develop a digital platform that enables clinical pathologists to request and provide second opinions, supported by AI algorithms and an innovative technique known as mass spectrometry imaging. The goal is to improve diagnostic accuracy and efficiency across borders. A demo version of the platform has been successfully developed by Pathomation, a new SME partner based in Antwerp. Pathomation stepped in after the bankruptcy of Cytomine and has already begun integrating AI algorithms into the platform using as proof-of-concept a commercially available AI model. Although their official involvement will be finalized in the next period, they are already contributing actively to the project. Clinical pathologists from partner hospitals have been identified and will begin testing the platform in the next phase. Progress has also been made toward developing two new AI algorithms, focusing on colorectal biopsies and lung cancer. In close collaboration with clinical pathologists, a cohort of samples is being selected. Hasselt University and Uniklinik Cologne are currently digitizing the associated pathology slides, which will serve as the basis for AI algorithm training. A PhD student at Hasselt University has already started algorithm research, applying existing AI techniques and conducting a literature review on AI algorithms for (cancer) diagnostics. He will also visit Uniklinik Cologne to gain hands-on insights into PD-L1 detection for routine pathology and the development of AI algorithms for pathology. A key strength of this project lies in its collaboration between different types of organisations across borders. Together with clinical pathologists, all the partners - UHasselt, UMaastricht, ULiège, Aspect Analytics and Uniklinik Köln - have together defined the scope of a legal study that addresses the cross-border sharing of digital pathology data, both for clinical use and AI development. They have worked together with pathologists from hospitals across the borders, from UZ Brussels, ZOL, Jessa Hospital, MUMC and Uniklinik Köln. These discussions have led to a set of key questions that will shape the upcoming tender for external legal expertise. To support the project, Hasselt University as lead partner led the effort to map out the data flows related to AI development, platform usage, and mass spectrometry imaging. All partners were involved in mapping the data flows in the different work packages. The clear data mapping has laid the groundwork for data and material transfer agreements, which will be finalized in the next project period. Overall, the project is firmly on track. Key technical, legal, and organizational foundations have been laid. Strong cooperation among academic, clinical, and SME partners across borders is already showing added value in terms of platform development, legal clarity, and AI innovation. The consortium will continue to meet regularly—both online and in person—to ensure timely progress. This project is on track in developing a digital platform designed to enable clinical pathologists to request and provide second opinions across borders. The primary objective is to enhance diagnostic accuracy and efficiency through the integration of AI algorithms and the advanced technique of mass spectrometry imaging (MSI). Main Achievements and Platform Output The cooperation has delivered several key outputs to date: - Digital Platform Launch and AI Integration: The platform, developed by Pathomation, was officially launched this summer, incorporating an AI marketplace. This is a significant output, as it allows pathologists to run AI algorithms from multiple vendors (currently including Mindpeak and Aiosyn) within a single, unified environment. This eliminates the need for parallel platforms for viewing, annotation, and analysis, representing a direct improvement in efficiency. Results are displayed immediately on the digital slide. - Demonstration Phase: A functional demonstration version is currently operational for the DigiPathConnect program, focusing on the development of core features such as case sharing for second opinions. - Knowledge Transfer and Data Preparation: A PhD student from UHasselt successfully visited UKK to gain expertise in clinical pathology. This collaboration is directly supporting the preparation of necessary data cohorts. Slides from UKK have also been digitized at UHasselt. Cohorts on colorectal biopsies (UKK) and another set of samples (UM) are being prepared and annotated, which is essential for algorithm training. Scientific Progress (Algorithm Development) Significant scientific progress has been made in algorithm development, which is a major output of the collaboration: - Colorectal Algorithm: Two main approaches, the SPARK model and vision-language models, have been developed. The SPARK model has been submitted to the journal Nature Medicine. The optimization of the single-cell algorithm is also underway. - Lung Cancer Algorithm: The full processing pipeline for this algorithm has been conceptualized. The first results will be presented as an e-poster at the ESMO AI & Digital Oncology Congress 2025 (November 12-14, 2025). The SPARK algorithm has also delivered initial results for PD-L1 prediction in lung adenocarcinoma. Added Value of Cross-Border Cooperation The collaborative effort across partner institutions has been critical in achieving multi-faceted progress: - Joint Expertise and Resource Sharing: The consortium leverages shared resources for digitization and transfers essential clinical expertise (e.g., the visit to UKK), accelerating both the technical and clinical aspects of the project. - Ethical and Legal Frameworks: The consortium is jointly finalizing a Joint Controller Agreement (JCA) to efficiently manage data transfers, eliminating the complexity of creating numerous separate data transfer agreements. Partners also collaborated to redraft the request for a legal study, resulting in three competitive offers, ensuring the project's legal compliance moves forward. - User-Centric Design: A comprehensive survey was collectively drafted and submitted for ethical approval (Hasselt University) to gather targeted input from clinical pathologists in the Euregion and Europe, ensuring the platform meets the needs for pathologists on the use for second opinions and AI tools. In summary, the project is on schedule, with the commercial AI algorithm implementation in the platform even being ahead of the initial plan. All activities are progressing as planned for the subsequent project period, supported by regular on-site consortium meetings.

Het bedrijf Cytomine en de Universiteit van Luik zijn beide partners in dit project en het Cytomine-platform is gekozen als centraal platform voor het bekijken en annoteren van klinische gegevens. De ervaring die is opgedaan en de ontwikkelingen die zijn gedaan in dit project in een onderzoekscontext zullen zeker ten goede komen aan ons project met betrekking tot samenwerking tussen pathologen in de klinische workflow, inclusief AI-integratie. We zullen onderzoeken of de in ons project verzamelde gegevens na afloop van het Bigpicture repository. (NL), The Bigpicture project aims at building the first European, ethical- and GDPR compliant central repository for quality-controlled digital pathology slides to boost the development of artificial intelligence in research settings. The University of Liège is a partner in this project, and the Cytomine platform, which is hosted by the University of Liège, has been chosen as the central platform for clinical data viewing and annotation. The experience gained, and developments done in this project in a research context will definitely benefit our project regarding collaboration between pathologists in clinical workflow, including AI integration. We will investigate whether the data collected in our project can be provided for research purposes through the Bigpicture repository after the project’s end. Ziel dieses Campus ist es, Unternehmen und Regierung-, Bildungs-, und Gesundheitseinrichtungen, Pflege- und Sozialdienstleister sowie Bürger miteinander zu verbinden. Einer seiner Schwerpunktbereiche ist die digitale und datengesteuerte Pflege. Ce campus vise à connecter les entreprises et les gouvernements, l’éducation, les établissements de santé, les prestataires de soins et de protection sociale et les citoyens. L’un de leurs domaines d’intervention est celui des soins numériques et basés sur les données. Par conséquent, notre projet peut se connecter à de nombreuses startups et entreprises plus matures dans le domaine de la santé (numérique), augmentant ainsi le nombre de PME que nous pouvons influencer positivement.

This campus aims to connect businesses and governments, education, health institutions, care and welfare providers, and citizens. One of their focus domains is digital and data-driven care. Therefore, our project can connect with many startups and more mature companies in (digital) healthcare, expanding the number of SMEs we can positively affect. (DE), The European Health Data Space, a project funded by the EU, aims to create a health ecosystem that improves the digital access of individuals to their personal health records and to repurpose this data for research, innovation, policy-making, and regulatory activities. All digital data generated in the hospital will be added to the EHDS. Therefore, all digital pathology images generated in this project can become part of the EHDS, which can only be accessed by the patient or anonymously by researchers, industry, or public institutions. Furthermore, once the EHDS project is finalized, data available through this ecosystem can be used to develop new AI-algorithms. Other Les soins de santé numériques et l’égalité d’accès aux soins de santé sont des priorités pour l’Union européenne.
Trois politiques de santé de l’UE sont pertinentes pour notre projet.
Premièrement, notre projet contribue à former des médecins à l’épreuve du temps pour la société numérique (Training Future-Proof Doctors for the Digital Society). En créant un réseau international de formation en pathologie et en élargissant l’utilisation de la pathologie numérique, nous formerons les futurs formateurs, c’est-à-dire les experts qui formeront à leur tour la prochaine génération de pathologistes et de scientifiques.
Nous contribuons également à améliorer les soins de santé dans les régions transfrontalières (Healthcare in Cross-Border Regions) en ouvrant les frontières en matière de soins de santé et en luttant contre le faible taux de santé dans l'Euregio par rapport à l'Europe. Nous partagerons également les bonnes pratiques concernant le partage sécurisé des données.
Enfin, notre projet soutient l'appel à l'action sur le dépistage du cancer colorectal dans l'UE (Call for Action on Colorectal Cancer Screening in the EU), car l'algorithme d'IA que nous développerons améliorera la précision et le délai d'exécution des biopsies effectuées lors des efforts de dépistage.
Other Ons project draagt bij aan verschillende Sustainable Development Goals (SDG's). In de eerste plaats dragen we bij aan SDG 3, die een gezond leven garandeert en welzijn bevordert voor iedereen, ongeacht leeftijd. Als ziekenhuizen samenwerken en kennis delen, krijgt iedereen dezelfde zorg, ongeacht het ziekenhuis. We zullen ook bijdragen aan SDG 8, om duurzame, inclusieve en duurzame economische groei te bevorderen. We verwachten dat ons project bedrijven (voornamelijk KMO's) in de sector zal helpen groeien door kennis en ervaring op te doen in DP, AI en andere geavanceerde technieken, wat zal leiden tot economische groei in de regio.
Other SALKturbo is an initiative from the Belgian province of Limburg, which aims to boost the economy in Limburg. They focus on four priorities and twelve ambitions. These four priorities are competitive, sustainable, digital and inclusive. Our project lines up within the digital priority, and more specifically is framed along the ambition line on "Using Digitization to become the Leading Health Lab in the Euregion".
Amongst others, the Health Campus Limburg, a future-proof hub in healthcare, and DigiPath, a project on Digital pathology, are SALKturbo projects. They are explained in further detail in C 2.6.
Other Le département fédéral de la santé publique (FOD Volksgezondheid / SPF Santé publique) en Belgique a lancé le plan national contre le cancer en 2008. À travers 35 actions, ils visent à évaluer et à améliorer la politique du cancer en Belgique, à travers de nouvelles stratégies et en fournissant un retour d'information.
Une de ces actions consiste par exemple à créer une banque de données sur les tumeurs, dans laquelle notre projet est impliqué, car les échantillons de cancer du poumon sont utilisés à partir de cette banque de données sur les tumeurs. Pour ce projet, un nouveau centre a été fondé, le centre de cancérologie (Kankercentrum / Centre du cancer), intégré à Sciensano. Ils participent à de nombreux projets, comme le projet EUCAIM sur les images médicales, expliqué plus en détail dans le C 2.6.
Other La Stratégie de Spécialisation Intelligente (S3), concept introduit par la Commission Européenne dans le cadre de son agenda de croissance et d'emploi, se positionne comme le cadre stratégique majeur en termes d’innovation en Wallonie. L' « Innovation pour une santé renforcée » est un des cinq Domaines d'Innovation Stratégique (DIS). Les ambitions affichées dans ce DIS sont (i) leader international en médicaments innovants, (ii) leader international en technologies diagnostiques et médicales et (iii) excellence en santé numérique à dimension humaine.
Notre projet s'inscrit parfaitement dans la priorité numérique des technologies de diagnostiques, des technologies médicales et de l(hôpital du futur (prévention et nouvelles organisations) fixées par la Région Wallone.
Other Similar to Belgium, In Germany "Der Nationale Krebsplan" was launched in 2008 to tackle the bottlenecks in cancer care and early cancer detection. They focus on four areas, being:
- Further development of earlier detection of cancer
- Further development of oncological care structures and quality of care
- Enable an efficient oncological treatment strategy
- Strengthen patient orientation
Our project matches this strategy, as we focus both on better detection of colorectal cancer in endoscopical biopsies, as well as aiming at improving the treatment response for immunotherapy in lung cancer.
Other In The Netherlands, the "Nederlands Kanker Agenda" was launched in 2023, as addition to the European Beating Cancer Plan, after starting the National Cancer Collective at the end of 2022. It consists of 20 goals, in four categories. One of these categories is diagnostics, treatment and follow-up, in which Diagnostics is a specific goal. In this goal, they have defined AI-applications as one of the solutions to improve diagnosis and prognosis. Molecular diagnostics is another solution mentioned for diagnostics. Therefore, our project again lines up with the Dutch ambitions on improved diagnostics and prognosis, both in the development of AI-algorithms, as well as the proof-of-concept on mass spectrometry imaging.
