Probleemstelling: De gezondheidszorgsystemen in de regio Maas-Rijn (MR) zijn al goed ontwikkeld & kwalitatief, maar gepolariseerd mbt transmurale zorg. Met andere woorden, zieke patiënten die op discrete momenten follow-up krijgen, zijn vatbaar voor frequente (her)ziekenhuisopnames, wat een overbelasting van de 2delijnszorgsystemen veroorzaakt, terwijl een follow-up binnen de 1stelijnszorg ontbreekt. Dit duidt op 2 grote valkuilen, nl. (1) de aanwezigheid van een 'black box” als het gaat om de gezondheid van patiënten thuis, & (2) de behoefte aan een nieuw zorgpad dat transmurale zorgtrajecten omsluit, afbakent & verbetert, waardoor een duidelijkere samenwerking en communicatie tussen de 1ste- en 2delijnszorg mogelijk wordt, incl. in grensoverschrijdende zorg. Omdat veel autoriteiten zich richten op het bieden van betere toegang tot gezondheidszorg, incl. RM & op zoek zijn naar innovatie in de gezondheidszorg, biedt dit project de perfecte gelegenheid om het pad te effenen voor de ontwikkeling & implementatie van hybride zorgpaden (HCP). Doelstelling: Ons project richt zich op de verbetering van de zorg voor zieke patiënten via (1) de integratie van monitoring op afstand (RM) mbv draagbare technologie om de patiënten thuis te monitoren en (2) het gebruik van deze gegevens en het verbeteren van de datastroom, samenwerking/communicatie tussen 1stelijns- (huisartsen, zorgorganisaties) & 2delijnszorg (ziekenhuis), grensoverschrijdende zorg & patiënten. Binnen RM@H wordt er gehouden met de verschillen en overeenkomsten tussen alle deelnemende regio’s (Vlaanderen, Wallonië, NL & DE) om een overstijgende visie op (transmurale) zorg te creëren met verbeterde datastromen en communicatie tussen de 1ste- & 2delijnszorg, & patiënten. RM@H wil een organisatorisch, technisch & juridisch raamwerk ontwikkelen voor de inzet van RM binnen de MR-regio, rekening houdend met de patiënt. Innovatie: Binnen RM@H zal via grensoverschrijdende samenwerking (WP1) de sterkstes en valkuilen geidentificeerd worden van het huidige klinische zorgtraject van volgende patiënten (pneumologie-, postoperatieve, hematologie- & psychiatrische patiënten) om de homo- en heterogeniteit eveneens te identificeren (WP1) & een nieuw theoretisch HCP formuleren, rekening houdend met de expertise van alle stakeholders (1ste-/2delijnszorg, en patiënt) (WP1). WP3 dient om de gezondheidszorg iteratief te transformeren & te digitaliseren via haalbaarheids-/proof-of-principle-proeven, incl. de organisatorische problemen van RM (WP2.1), de eisen aan dashboarding (WP2.2), de impact van voorspellende modellen (WP2.3) en de toegevoegde waarde van woonomstandigheden (WP2.4). In WP4 wordt een juridisch raamwerk ontwikkeld dat de vereisten identificeert om medische gegevens transmuraal te verwerken en uit te wisselen, om RM mbv fitness vs medische apparaten mogelijk te maken & om voorspellende modellen te gebruiken om deze in deze en toekomstige zorgtrajecten in te bedden. Impact: RM@H verbetert transmurale zorg via RM door betere betrokkenheid van de 1stelijnszorg, transitie van ziekenzorg naar gezondheidszorg (d.w.z. voorspellende modellen), inzichten in de ziekteprogressie thuis, met respect voor lokale en het EU-recht, en dus in het algemeen de toegang tot en de zorg voor de patiënt. Als dit project niet zou worden gehonoreerd, zouden verschillende kansen, zoals hierboven beschreven, gemist worden, evenals het huidige momentum dat aanwezig is om innovatieve veranderingen na te streven.

Énoncé du problème: Les systèmes de santé dans la région Meuse-Rhin (MR) sont bien implantés et hautement qualitatifs, mais polarisés en matière de soins transmuraux. Ainsi, les patients bénéficiant d’un suivi de leur maladie à des moments discrets sont sujets à des (ré)hospitalisations fréquentes, créant une surcharge pour les systèmes de soins secondaires, tout en manquant d’un suivi clair au sein des soins primaires. Cela indique 2 pièges majeurs dans les systèmes actuels: (1) le manque de données concernant la santé et le bien-être des patients à domicile, (2) la nécessité de nouveaux trajets de soins qui encadrent et améliorent les soins transmuraux, permettant une coopération et une communication plus claires entre les soins primaires et secondaires, y compris les soins transfrontaliers. Objectif : Notre projet se concentre sur l'amélioration des soins aux patients via (1) l'intégration de la télésurveillance à domicile et (2) l'utilisation de ces données et l'amélioration du flux de données, de la coopération/communication entre soins primaires (médecins généralistes, organismes de soins) et secondaires (hôpitaux), transfrontaliers et les patients eux-mêmes. L'un des principaux atouts de RM@H réside dans la prise en compte des différences et des points communs entre toutes les régions participantes (Flandre, Wallonie, Pays-Bas et Allemagne) pour créer une vision des soins (transmuraux) avec des flux de données et une communication améliorée entre les soins primaires et secondaires, et les patients. Globalement, RM@H souhaite développer un cadre organisationnel, technique et juridique pour le déploiement de la télésurveillance au sein de la région MR en tenant compte du patient. Innovation: RM@H identifiera via une collaboration transfrontalière (WP1) les forces et les pièges des trajets de soins actuels de groupes de patients prédéfinis (pneumologie, post-opératoire, hématologie, patients psychiatriques) pour identifier l'homo - et hétérogénéité (WP1) et formuler un nouveau trajet théorique prenant en compte l'expertise de tous les acteurs (soins primaires et secondaires, patients) (WP1). WP3 sert à transformer et à numériser le trajet de manière itérative via des essais de faisabilité/preuve de principe, incluant les résultats itératifs sur les difficultés organisationnelles pour la télésurveillance (WP2.1), les exigences en matière de tableaux de bord (WP2.2), l'impact de la modélisation prédictive (WP2.3) et la valeur ajoutée des conditions d'habitat (WP2.4). De plus, dans le WP4, un cadre juridique identifiant les exigences relatives au traitement et à l'échange de données médicales de manière transmurale sera développé, considérant la pertinance des dispositifs wellness par rapport aux dispositifs médicaux et l'utilisation des modèles prédictifs pour les intégrer dans ce parcours de soins et dans les futurs. Impact : RM@H permet une amélioration transfrontalière des soins transmuraux via l'introduction de la télésurveillance, une meilleure implication des soins primaires, la transition des soins curatifs aux soins préventifs (modèles prédictifs), une compréhension de la progression de la maladie à domicile, le respect des lois et le droit de l'UE, et ainsi améliore globalement la qualité et l'accès aux soins. Si ce projet n’était pas accordé, plusieurs opportunités, telles que décrites ci-dessus, seraient manquées, ainsi que la dynamique actuelle en place pour poursuivre un changement innovant.

Problem statement: The current healthcare systems in the Meuse-Rhine (MR) region are already well established and highly-qualitative, yet fairly polarized when it comes to transmural care. In other words, diseased patients that receive disease follow-up only at discrete moments are prone to frequent (re)hospitalizations creating an overload to the secondary care systems, whilst lacking a clear follow-up within primary care. This indicates 2 major pitfalls in the current systems being (1) the presence of a black box when it comes to the patients’ health and wellbeing at home, and (2) the need for a new care path that encloses, delineates and improves transmural care, allowing clearer cooperation and communication between primary and secondary care, incl. in particular cross-border care. As many authorities focus on providing better access to health, including RM and are searching for innovation in healthcare, this project poses as the perfect opportunity to pave the path for the development and implementation of hybrid care paths (HCP). Objective: Our project focuses on the improvement of care of diseased patients via (1) the integration of remote monitoring (RM) using wearable technology to monitor the patients at home and (2) using this data and improving the dataflow, cooperation/communication between primary (GP’s, care organizations) & secondary (hospitals) care, cross-border & patients. A major strength of RM@H is that differences & commonalities between all participating regions (Flanders, Wallonia, NL & DE) are taken into account to create a transcending vision on (transmural) care with improved dataflows & communication between primary & secondary care, & patients. Overall, RM@H wants to develop an organizational, technical & legal framework for the deployment of RM within the MR region taking into account the patient. Innovation: RM@H will identify via cross-border collaboration (WP1) the strengths and pitfalls of the current clinical care path of pre-defined patient groups (i.e., pneumology, post-surgical patients, haematology & psychiatric patients) to identify the homo- and heterogeneity (WP1) and formulate a novel theoretical HCP taking into account the expertise of all stakeholders (primary & secondary care, & patient) (WP1). WP3 serves to transform and digitalize the HCP iteratively via feasibility/proof-of-principle trials, incl. the iterative findings in the organizational difficulties of RM (WP2.1), the requirements on dashboarding (WP2.2), the impact of predictive modelling (WP2.3) and the added value of housing conditions (WP2.4). Moreover, in WP4 a legal framework identifying the requirements to process and exchange medical data transmurally, to enable RM using fitness vs medical devices and to use predictive models will be developed to embed them in this and future care paths. Impact: RM@H enables a cross-border improvement in transmural care via the introduction of RM, improved involvement of primary care, transition from sick care to health care (i.e., predictive models), insights in the disease progression at home, respecting local and EU law, and thus overall ameliorating access and care for the patient. If this project would not be granted, several opportunities, as described above, would be missed, as well as the current momentum that is in place to pursue innovative change.

Problemstellung: Die Gesundheitssysteme der Region Maas-Rhein (MR) sind gut & qualitativ etabliert, in der transmuralen Versorgung jedoch polarisiert. Patienten, die nur zu bestimmten Zeitpunkten nachbehandelt werden, neigen zu häufiger (Re-)Hospitalisierung, die eine Überlastung der Sekundärversorgungssysteme zur Folge hat, während es in der Primärversorgung an einer Nachsorge mangelt. Dies deutet auf zwei große Fallstricke der Systeme: (1) das Vorhandensein einer BlackBox, bzgl. Gesundheit/Wohlbefinden von Patienten Zuhause & (2) Die Notwendigkeit eines neuen Versorgungssystems, das die transmurale Versorgung einschließt, abgrenzt & verbessert &e eine klarere Zusammenarbeit/Kommunikation der Primär-& Sekundärversorgung auch grenzübergreifend, ermöglicht. Viele Behörden konzentrieren auf die Verbesserung des Zugangs zur Gesundheitsversorgung in der Region MR & suchen nach Innovationen im Gesundheitswesen. Das Projekt bietet die ideale Möglichkeit, den Weg für die Entwicklung & Umsetzung von hybriden Versorgungspfaden (HCP) zu ebnen. Ziel: Das Projekt konzentriert sich auf die Verbesserung der Versorgung von Patienten durch (1) die Integration von Remote Monitoring (RM) mit tragbarer Technologie zur Betreuung/Überwachung der Patienten Zuhause & (2) die Nutzung dieser Daten & die Verbesserung des Datenflusses, der Kooperation/Kommunikation zwischen primärer (Hausärzte, Pflegeorganisationen) & sekundärer (Krankenhäuser) Versorgung, einschließlich grenzübergreifender Versorgung von Patienten. Eine große Stärke von RM@H ist, dass Unterschiede & Gemeinsamkeiten der Regionen (Flandern, Wallonien, NL & DE) berücksichtigt werden, um eine transzendente Vision der (transmuralen) Versorgung mit verbesserten Datenflüssen zu schaffen. Insgesamt will RM@H einen organisatorischen, technischen & rechtlichen Rahmen für den Einsatz von RM in der MR-Region unter Berücksichtigung des Patienten entwickeln. Innovation: RM@H wird in grenzübergreifende Zusammenarbeit (WP1) die Stärken & Fallstricke des aktuellen klinischen Versorgungspfads für vordefinierte Patientengruppen (z.B. Pneumologie, postoperative Patienten, Hämatologie & psychiatrische Patienten) identifizieren, um die Homo- & Heterogenität (WP1) zu identifizieren & ein neue theoretische HCP mit Berücksichtigung der Expertise von o.g. Stakeholdern zu formulieren (WP1). WP3 dient der iterativen Transformation & Digitalisierung des HCP mittels Machbarkeits-/Proof-of-Prinzip-Versuchen, inkl. iterativer Befunde organisatorischer Schwierigkeiten von RM (WP2.1), Anforderungen an das Dashboarding (WP2.2), Auswirkungen von Predictive Modeling (WP2.3) & Mehrwert v. Wohnbedingungen (WP2.4). In WP4 wird ein Rechtsrahmen entwickelt, der die Anforderungen an transmurale Verarbeitung & Austausch von medizinischen Daten, die Ermöglichung von RM mit Fitness vs. Medizinprodukten & den Einsatz von Vorhersagemodellen zur Einbettung in diese & zukünftige Versorgungspfade festlegt. Wirkung: RM@H ermöglicht eine grenzübergreifende Verbesserung der transmuralen Versorgung durch die Einführung von RM, eine bessere Einbeziehung der Primärversorgung, den Übergang von der Kranken- zur Gesundheitsversorgung (d.h. prädiktive Modelle), Einblicke in den Krankheitsverlauf Zuhause unter Einhaltung lokaler/ EU-Gesetze & so eine Verbesserung des Zugangs & der Versorgung des Patienten. Ohne Bewilligung des projektes würden mehrere der o.g. Möglichkeiten versäumt, wie die derzeitige Dynamik, die für den innovativen Wandel vorhanden sind

Rekening houdend met de huidige situatie per ziekte, zullen nieuwe GZP worden gevormd waarin RM en de transmurale verschuiving tussen 1e en 2e lijnszorg vervat zit. Via co-creatiesessies wordt de grensoverschrijdende problematiek/ziekte aangekaart. Ontwikkeling van de bouwsteen voor de nieuwe hybride zorgpaden op: organisatie, technisch, dashboard (WP1). Voorspellende modellen en de impact van woonomstandigheden op de gezondheid zullen respectievelijk worden ontwikkeld en onderzocht. Door HZP voor verschillende ziektes iteratief te vormen geeft dit een generieke aanpak over ‘hoe’ HZP te implementeren. De generieke aanpak (I.e. HZP) is onafhankelijk van de ziekte en technologie, en wordt grensoverschrijdend toegepast. Een raamwerk dat voldoet aan EU- en lokale regels, dat inzichten in gegevensverwerking, monitoring op afstand en voorspellende modellen integreert en alle resultaten van de werkpakketten benut voor een raamwerk dat kan worden vertaald naar zorgpaden.

Basés sur la prise en charge actuelle, de nouveaux trajets de soins intégrant la télésurveillance seront élaborés impliquant les acteurs de 1re et 2e lignes. Grâce à la co-création, les problèmes transfrontaliers dans ces maladies seront considérés. Développement des briques de base des nouveaux parcours de soins hybrides : organisation, aspects techniques et les tableaux de bord (WP1). Des modèles prédictifs et l'impact des conditions du logement sur la santé seront développés et étudiés. Les trajets de soins pour différentes maladies sont formés de manière itérative pour aboutir à une approche générique sur la manière de les mettre en œuvre. Cette approche générique est indépendante de la maladie/technologie/regio. Cadre conforme aux lois européennes et locales, intégrant des informations sur le traitement des données, la télésurveillance et les modèles prédictifs, et exploitant les résultats de tous les WPs pouvant être traduit en trajets de soins.

Expected Achievements (EN):

Taking into account the current situation per disease, new HCP will be drafted incorporating RM and a transmural shift between 1ry and 2ndry care. Via co-creation sessions attention will be given to the cross-border problems/disease. Developing the building block of the new hybrid care paths on: organization, technical, dashboard (WP1). Predictive models & the impact of housing conditions on health will be developed and investigated, respectively. HCPs for different diseases are iteratively formed to ultimately lead to a generic approach on ‘how to’ implement HCPs. This generic approach constitutes of being independent of the disease and technology proposed, and can be applied cross-border. Develop a framework, compliant with EU and local laws, integrating insights on data processing, remote monitoring, and predictive models, and leveraging all work packages outcomes for a novel framework that can translated into care paths.

Mit Berücksichtigung der aktuellen Situation pro Krankheit wird ein Versorgungspfad entworfen, der die Fernbetreuung & transmurale Verschiebung zwischen 1.& 2. Versorgern umfasst. Auf grenzübergreifende Probleme/Krankheit wird in Gruppen eingegangen. Entwicklung der Bausteine der neuen hybriden Pfade für: Organisation, Technik, Dashboard (WP1). Vorhersagemodelle und Auswirkungen von Wohnbedingungen auf die Gesundheit werden entwickelt und untersucht. Versorgungsspfade für verschiedene Krankheitbilder werden iterativ gebildet, um einen generischen Ansatz für deren Umsetzung "HowTo“ zu generieren. Der generische Ansatz ist unabhängig von Krankheit/Technologie und wird grenzüberschreitend angewandt. Entwicklung EU & lokalrechtlich konformer Rahmen mit Erkenntnissen zu Datenverarbeitung, Fernüberwachung Vorhersagemodellen etc. durch Nutzung der Ergebnisse aller Arbeitspakete zur Umsetzung HCPs.

European Health Data Space (DE), European Health Data Space (NL), European Health Data Space (FR), European Health Data Space (EN) | De EHDS heeft 3 doelstellingen; (a) Individuen empoweren door betere digitale toegang tot hun persoonlijke gezondheidsgegevens en het vrije verkeer van mensen ondersteunen, (b) de data-economie ontketenen door een interne markt voor digitale gezondheidszorgdiensten te bevorderen en (c) strikte regels opstellen voor het gebruik van niet-identificeerbare gezondheidsgegevens van individuen voor onderzoek, innovatie, beleidsvorming en regelgevingsactiviteiten. Dit project zal de EHDS doelstellingen verder uitwerken door een implementatie van telemonitoring in een interregionale context mbv één softwaresysteem in de verschillende zorgpaden te onderzoeken. Hierdoor wordt innovatief onderzoek mogelijk gemaakt, waarin de impact op de efficiëntie van de gezondheidszorg en de kwaliteit van de zorg worden onderzocht. De problemen op het gebied van regelgeving en wetgeving worden ook aangepakt die voortvloeien uit een interregionale samenwerking van zorginstellingen. (NL), EHDS porte 3 objectifs ; (a) Donner aux individus un meilleur accès numérique à leurs données personnelles de santé, (b) libéraliser l'économie des données en favorisant un véritable marché pour les services de santé numériques et (c) établir des règles strictes pour l'utilisation des données personnelles de santé pour les activités de recherche, d’innovation, d’élaboration de politiques et de réglementation.
Ce projet approfondira les objectifs de l'EHDS au travers de la mise en œuvre de la télésurveillance dans un contexte interrégional à l'aide d'un logiciel connecté aux DMIs des partenaires cliniques et accessible aux acteurs des différents trajets de soins. Cela facilitera une étude clinique innovante, étudiant l’impact sur l’efficacité des soins et la qualité des soins de santé. Ce projet répondra spécifiquement aux préoccupations réglementaires et législatives soulevées par une collaboration interrégionale des établissements de santé. (FR), EHDS hat 3 Ziele: a) Befähigung des Einzelnen durch besseren digitalen Zugang zu seinen personenbezogenen Gesundheitsdaten & Unterstützung der Freizügigkeit, b) Entfaltung der Datenwirtschaft durch Förderung des Binnenmarkts für digitale Gesundheitsdienste & c) Festlegung strenger Regeln für Nutzung nicht identifizierbarer Gesundheitsdaten von Einzelpersonen für Forschungs-, Innovations-, Politik- & Regulierungszwecke.
Das Projekt arbeitet die Ziele des EHDS weiter aus, indem Fernüberwachung &-betreuung in einem interregionalen Kontext mittels eines Softwaresystems implementiert wird, das mit EHRs der klinischen Partner verbunden &für Akteure diverser Versorgungspfade zugänglich ist. Dies ermöglicht eine innovative klinische Studie, die die Auswirkungen auf Effizienz & Qualität der Gesundheitsversorgung untersucht. Zudem befasst sich das Projekt gezielt mit regulatorischen& legislativen Fragen, die sich aus einer interregionalen Zusammenarbeit von Gesundheitseinrichtungen ergeben. This project will further elaborate the goals of the EHDS through an implementation of remote monitoring in an interregional context using one software system that is connected to the EHRs of the clinical partners and accessible for the stakeholders in the various care path. Doing so will facilitate an innovative clinical study, investigating the impact on healthcare efficiency and quality of care. Furthermore, this project will specifically address the regulatory and legislative concerns raised by an interregional collaboration of healthcare institutes. De nadruk ligt op COPD en hartziekten om verergering van de ziekte te voorkomen. Omgevingsfactoren zoals luchtvervuiling (binnen en buiten), straatlawaai en straatverlichting worden verzameld door middel van de Expanseseeker en een draagbare sensor.
Games for Health, partner in dit grote internationale Horizon 2020-consortium, heeft de Expanseseeker ontwikkeld en zal de ervaringen uit dit project benutten ten behoeve van RM@H. (NL), Das Expanse-Projekt hat das Ziel, menschliche Exposome mit Umweltexposomen abzugleichen. Der Fokus liegt auf COPD und Herzkrankheiten zur Vorbeugung von Exazerbationen der erkrankten Person. Umweltfaktoren wie Luftverschmutzung (Innen- und Außenbereich), Straßenlärm, Straßenblitze werden mit dem Expanseseeker und einem tragbaren Sensor erfasst.
Games for Health, Partner des großen internationalen Konsortiums Horizon 2020, hat den Expanseseeker entwickelt und wird die Erfahrungen aus diesem Projekt zum Nutzen von RM@H nutzen. (DE), The Expanse project has the objective to match human exposomes with environmental exposomes. Focus is on COPD and Cardiac Disease for the prevention of exacerbation of the diseased. Environmental factors like air pollution (indoor and outdoor), street noise, street lightning are collected by means of the Expanseseeker and a portable sensor.
Games for Health, partner in this major Horizon 2020 international consortium, developed the Expanseseeker and will leverage experiences of this project for the benefit of RM@H. Eines der Konsortiumsmitglieder, Games for Health, beteiligte sich am CUOREMA-Projekt und wird seine bisherigen Erfahrungen aus Co-Creation und europäischen finanzierten Projekten nutzen, um das Patient-flow Design im aktuellen Projekt zu unterstützen. Il offre un environnement d'apprentissage interactif et innovant aux étudiants, aux enseignants et aux professionnels du domaine, axé sur la recherche et l'apprentissage transdisciplinaires dans le domaine de la santé. Le laboratoire de simulation et d'innovation THINK³ est hébergé par l'Université de Hasselt (Lead Partner) et POM Limburg.
L'un des projets sélectionnés au sein du laboratoire de simulation et d'innovation THINK³ est le « Projet d'amélioration des soins transmuraux pour les patients BPCO ». Le projet actuel contribuera directement à cet objectif de recherche en permettant des soins transmuraux via la télésurveillance des conditions d'habitat et des paramètres vitaux. De plus, la Croix Jaune et Blanche, qui fait partie de ce projet, a également montré son intérêt à notre projet (LOI en annexe). Het THINK³ simulatie- & innovatielab wordt gehost door Universiteit Hasselt (Lead Partner) en POM Limburg.
Eén van de geselecteerde projecten binnen het THINK³ simulatie- & innovatielab is ‘Verbeteringsproject van Transmurale Zorg voor COPD-patiënten’. Het huidige project zal rechtstreeks bijdragen aan dit onderzoeksdoel door het mogelijk maken van transmurale zorg via monitoring op afstand van de huisvestingsomstandigheden en vitale parameters. Bovendien heeft Wit-Gele Kruis, onderdeel van dit project, ook toewijding getoond aan ons project (LOI in Appendix). It provides an interactive and innovative learning environment for students, teachers, and professionals from the field, focusing on transdisciplinary research and learning in healthcare. The THINK³ simulation & innovation lab is hosted by Hasselt University (Lead Partner) and POM Limburg.
One of the selected projects within the THINK³ simulation & innovation lab is “Improvement Project of Transmural Care for COPD patients”. The current project will directly contribute to this research goal through the enablement of transmural care via remote monitoring of housing conditions and vital parameters. Moreover, Wit-Gele Kruis part of this project has also shown dedication to our project (LOI in Appendix). Het huidige project maakt gebruik van eerdere marktanalyses, ontwikkelingsproces en kennis, en past deze aan en optimaliseert deze aan de huidige omstandigheden. Daarnaast probeert het deze kennis uit te breiden en over te dragen naar praktische toepassingen, zowel in de eerste als in de tweede lijn. Dit omvat een strategische overdracht van inzichten die zijn verworven in de ziekenhuiscontext naar het domein van RM thuis, waardoor een uitgebreider begrip van patiëntenzorg wordt bevorderd dat verder gaat dan de traditionele gezondheidszorgomgeving. Das aktuelle Projekt greift auf bisherigen Marktanalysen, Entwicklungsprozessen und Erkenntnissen zurück, passt diese an die aktuellen Gegebenheiten an und optimiert sie. Daneben soll dieses Wissen erweitert und in die Praxis umgesetzt werden, sowohl in der Primär- als auch in der Sekundärversorgung. Dies beinhaltet einen strategischen Transfer von Erkenntnissen aus stationären Kontexten in den Bereich der häuslichen RM, um ein umfassenderes Verständnis der Patientenversorgung über die traditionelle Gesundheitsversorgung hinaus zu fördern. The current project taps into previous market analysis, development process, and knowledge, adjusting and optimizing it to the current conditions. Next to that, it seeks to expand and transfer this knowledge into practical applications, including both primary and secondary care settings. This involves a strategic transfer of insights gained in in-hospital contexts to the realm of RM at home, fostering a more comprehensive understanding of patient care beyond traditional healthcare settings. Other Auf supranationaler Ebene wird der Hauptbeitrag des Projekts zu den im Europäischen Gesundheitsdatenraum (EHDS) postulierten Zielen sein. Erstens wird die Implementierung eines grenzüberschreitenden Fernüberwachungs-/-versorgungssystems Patienten und Gesundheitsdienstleistern in klinischen Iterationen ermöglichen, unabhängig eines Standort auf die Messungen von Vitalparametern zuzugreifen. Zusätzlich untersucht das Projekt den optimale Austausch von Gesundheitsdaten zwischen dem Softwaresystem, das die Fernüberwachung implementiert und den EHRs.
Auf nationaler Ebene unterstützt das Projekt für Belgien die strategischen Ziele der Provinz Limburg, die in den letzten Jahren umfangreich in Projekte zum Aufbau & zur Validierung von Know-how im Gesundheitswesen investiert hat (z.B. Health Campus, Remote Clinical Monitoring Center). Darüber hinaus strebt die örtliche Primary Care Authority (ELZ Herkenrode) in ihren Politikplänen (2024-2026) eine verstärkte Einbeziehung der integrierten und transmuralen Pflege an.
Die Niederlande haben das „Integrale Zorgakkoord“ im September 2022 genehmigt. Eines der Ziele dieses Dekrets ist es, den effizienten Datenaustausch zwischen Gesundheitsdienstleistern und Patienten zu erhöhen und die Vorteile der hybriden Versorgung durch digitale Technologien zu nutzen, die den Umzug in die häusliche Pflege nach Möglichkeit fördern.
Im Dezember 2023 hat das Bundesministerium für Bildung und Innovation die „Zukunftsstrategie Forschung und Innovation“ veröffentlicht. Speziell für Nordrhein-Westfalen hat das EFRE/JTF-Programm NRW 2021-2027 „Innovation des Gesundheitswesens, Innovative Medizin, Gesundheit und Life Science“ als einen der sieben strategischen Wachstumsfelder ausgewählt.
Daher kann das Projekt, wie in Abschnitt 2.2 beschrieben, eine engere Zusammenarbeit zwischen den teilnehmenden Regionen fördern, die alle die hybride Versorgung als strategischen Politikbereich hervorgehoben haben, um grenzüberschreitende Innovationen zu fördern.
