Projects objective was to create a methodology for the involvement of elderly people living at home and in housing contexts in order to reduce loneliness through the use of IT tools capable of connecting people within an on-line and in-presence community in which they could develop their mental and social resources in order to maintain their health. The project involved three PIlots in Alpine and urban contexts. - niCE-Life Project aims to foster social inclusion and care coordination of the elderly with cognitive medium-low deficits, including Alzheimer's and Parkinson's diseases and other chronic diseases. It will develop an innovative health care model by using progressive, key enabling technologies such as sensor technologies, ICT and data analysis techniques. (EN), I-CARE SMART - Interreg Central Europe (EN) | Innovation Ecosystem for smart elderly care (https://www.interreg-central.eu/Content.Node/I-CARE-SMART.html) implemented three Thematic Working Groups (Senior Engagement, Business Engagement, Co-creation and Open Innovation Methods) to develop different perspectives in order to empower the partners and Task4 members in strengthening co-creation in elderly care. The connections with this approach will create with the PROCAREFUL project huge potentials for creating innovation through a better understanding of user needs by promoting integration between public and private services. (EN), digitalLIFE4CE - Interreg Central Europe (EN) | Digital Life for Central Europe – Fostering innovation in integrated healthcare systems solutions (https://www.interreg-central.eu/Content.Node/digitalLIFE4CE.html). The project did research and promoted best practice cases in so-called ‘Healthcare Excellence Spots’ and investigated ways to increase investment in digital health to boost innovation in healthcare systems with a need for integrated care and the PROCAREFUL model scales it up with a new organisation hybrid-care model. (EN), HoCare 2.0 - Interreg Central Europe (EN) | In this project they engage the members of Quadruple Helix - SMEs, public institutions, research institutions and citizens in the development of new innovative health and social services. ISRAA worked on the topic of development support for elderly - mainly upgrading the services for elderly and remote care. The co-creation strategies adopted by the PROCAREFUL model can empower users and engaging them into the development process.BSC Krany (SLO) as AP it will be support the HoCare 2.0 scale up. (EN), IDEAHL- Horizon Europe (EN) | Improving Digital Empowerment for Active Healthy Living where ISRAA is a partner aims to create new models and approaches of (digital) health literacy intervention through the co-creation of a comprehensive and inclusive EU (d)HL Strategy. With the PROCAREFUL model the idea is to empower EU citizens in Central Europe area in using digital tools and taking a more active role in the management of their own health and well-being. (EN), SOCATEL - Horizon 2020 (EN) | A multi-stakeholder co-creation platform for better access to long-term care services (https://www.socatel.eu/). There will be scaling up in the use of digital platforms for citizens’ co-creation around social and health related topics. With the PROCAREFUL model there is an improvement on the efficiency, transparency and responsiveness of the social services. ISRAA as a partner in the project can provide the SOCATEL platform. (EN), PROCare4Life - Horizon 2020 (EN) | PROCare4Life is an EU funded project proposing an integrated, scalable and interactive care ecosystem, easily adapted to several chronic diseases, care institutions and end-user needs. Consisting of 14 partners from 6 EU Member states, PROCare4Life promotes quality of life for older adults. This would benefit individuals, families and health professionals. Wohlfahrtswerk as one of the pilot sites has the objective:
- · Make a good alignment between user needs, requirements and technical development through testings and co-production/participatory design sessions.
- · To test the feasibility (including people’s satisfaction, people’s perception of utility and safety) of the general recommendation provided by the system.
- · To evaluate the usability of the monitoring devices (measurements made by sensors and wearable and fixed devices, application functionality) in the real life of older people with PD, AD and OD, their caregivers and healthcare professionals.
- · To collect data, during iterative testing, that feed the system and allows developing other functionalities of the system.
- · To evaluate the characteristics of the metrics generated by the PROCare4Life platform in real-life conditions.
- To get conclusions on the benefits of PROCare4Life approach towards integrated care and improved well-being and quality of life of older people, indicating possible ways to address frailty, cognitive impairment and prevention of neurodegenerative and other chronic diseases across different settings and institutions. (EN), TechCare - Erasmus+ (EN) | TechCare – Training on the adaptability of assisted living technologies in home and community care is a three year project funded by the Erasmus+ Program by the European commission and was developed by 5 partners from our countries (Belgium, Greece, Germany and Spain). The main objective of the TECH CARE project is to promote an ethically sound and practically viable adaptation of assisted living technologies in long term care. The project’s contribution to this objective is to train carers as evaluators and primary level counselors upon the ethical and practical challenges that determine the acceptability of assisted living technologies (ALTs) into home and community care. (EN), ITHACA - Interreg Europe (EN) | CEI Partner - the project involves 9 EU regions with a shared ambition to accelerate the scaling up of smart health and care innovation that can support active and healthy ageing. The main goal is to enhance regional and interregional ecosystems on smart health sector.

Contribution to wider strategies and policies:
- EuropeanGreenDeal Implementing novel models for the provision of e-care and e-health proactive care services can help significantly to reduce emissions from vehicles related to the delivery of services: proactive remote health models are likely to promote better health and less need for healthcare workers to move to remote areas thus reducing carbon fossil consumption and emissions. Relying on technologies available on the market will also allow the delivery of innovative solutions without consuming new resources.
- TerritorialAgenda2030 The overall analysis from pilot implementations will contribute to tackling the needs and the features of different areas of the CE region, fostering more than just development. This can be achieved by delivering a model capable of showing how digital and remote access to care services can reduce inequalities between people and places. Delivering a tested implementation manual and respective policy recommendations will help to scale up the uptake of innovative care and health solutions, thus realising the potential of a wide cross-border and regional cooperation.
- Making services more accessible across cross-border regions is expected to deliver a scalable solution capable of realising its potential and capable of facing growing social challenges.
- EUStrategyDanubeRegion Project activities and outputs will be a potential contribution to the PA 10 of the EUSDR (Institutional capacities & cooperation, priorities 1.1 and 1.2.). Digitalisation of access to health care and services through ICT can promote a more efficient delivery of related services and opportunities. At the same time the outputs and knowledge delivered from the project will be a fruitful source to understand key actions and actors to co-design actions and policies for digitalisation of services.
- EUStrategyBalticSeaRegion Outputs of the project are expected to contribute significantly to the EUSBSR PA Health and PA Innovation by both creating a methodology capable of supporting and promoting proactive healthy ageing and counteracting the negative effects of demographic change. Through the local implementation of the model, it will be possible to shape and promote policies aiming at deploying innovative home care system adaptations, thus reducing the societal costs of the ageing population through adaptation.
- EUStrategyAlpineRegion The outcomes of the project will contribute to developing and implementing care-related innovative Service of General Interest, especially in remote, rural and less developed areas, where services are less available and the impact of the ageing population and its related needs is stronger. The project outputs will contribute to reducing disparities in these regions, making service accessible, putting the users and their needs at the centre, improving quality of life in CE countries.
- Other The cross-border, policy-specific and context-based approach to the delivery of the proactive care model is connected with the ESPON 2030 TAP on a “Perspective for all people and places”, by addressing social inequalities, vulnerabilities related to existing social challenges, disparities and service availability also in remote, rural, peripheral and distant areas of the CE initiative.
- Other The implementation manual for the implementation of proactive-digital models is also in line with the Cohesion Policy objectives (21-27) a more Social and Inclusive Europe and a Europe closer to citizens, by creating concrete opportunities to act at territorial level in order to make core services available also in less connected regions, thus promoting better quality of life.
- Other Since the outputs produced by the project are meant to strengthen the capacity of territorial authorities and policy makers to uptake and scale up digital care and health solutions also through staff training and need analysis of institutions, results from the project are expected to be in line with SO3 of the WHO Global strategy on digital health. The use of solutions that can be deployed in different areas, including rural and remote, makes the project’s outputs and activities also in line with SO4.
