The project, 'Proactive Health Initiative: Building a Healthier Future Together,' addresses critical healthcare challenges in Romania and the Republic of Moldova. Both countries grapple with aging populations, exacerbated by workforce emigration, healthcare underfinancing, rural-urban healthcare disparities, and medical professional migration. Persistent challenges include poverty, hindering access to essential resources, and an underdeveloped socio-medical culture. This results in a population with high mortality rates and life expectancy below the EU average. The objective of the PREVENT project is to enhance healthcare access and resilience in underserved regions of Romania and Moldova. It envisions a transformative change in the current healthcare landscape, emphasizing preventive medicine, bolstering primary health care, reducing disparities, and fostering a shift towards community-based care. In order to tackle the challenges mentioned above, the initiative seeks to bridge existing gaps in healthcare accessibility, especially in rural areas where vulnerable populations, notably the elderly, face challenges in receiving quality medical services. By prioritizing preventive medicine, the project aims to instill a cultural shift towards proactive health management, steering away from reactive care. The emphasis on promoting primary health care aims to strengthen the foundation of healthcare services, ensuring that communities have accessible and comprehensive medical support. A key aspect of the expected change is the transition to community-based care, empowering local communities to actively participate in their health management. Through cross-border collaboration, joint training programs, and the exchange of experiences among medical staff, the project aims to elevate the quality of medical services and create a sustainable model for healthcare delivery. The primary outputs of the PREVENT project include the development and implementation of effective tools for health monitoring campaigns, the creation of robust local volunteer networks, comprehensive training sessions for project personnel and volunteers, impactful Healthy Communities Campaigns, informative Health Education Seminars, and a transformative Health Caravan delivering essential healthcare services to vulnerable populations. These outputs aim to enhance healthcare accessibility, instill preventive healthcare practices, and strengthen community engagement. The beneficiaries encompass a wide spectrum, ranging from elderly individuals in rural and disadvantaged small urban areas who will receive improved health assessments and support, to local communities benefitting from sustainable engagement and knowledge sharing. Moreover, project personnel and volunteers will acquire valuable skills, contributing to long-term healthcare improvements.
