De Wereldgezondheidsorganisatie riep in 2014 antibioticaresistentie uit tot één van de grootste bedreigingen voor de volksgezondheid. Zonder aanvullende maatregelen is voorspeld dat in 2050 meer mensen door antibioticaresistente ziekteverwekkers overlijden dan door kanker. Ondanks de toenemende resistentie zijn op korte termijn echter geen nieuwe antibiotica op de markt te verwachten. ‘i-4-1-Health’ - geleid door het Amphia ziekenhuis met hoofdlocatie in Breda - wil resistentie bij gezonde burgers, patiënten en de varkens- en pluimveesector inzichtelijk maken. Projectverantwoordelijke professor Jan Kluytmans legt uit: Grensoverschrijdende samenwerking is daarbij noodzakelijk aangezien de verspreiding van antibioticaresistente ziekteverwekkers niet stopt bij de landsgrenzen. Het is specifiek relevant in het grensgebied Vlaanderen-Nederland gezien het nauwe handels- en personenverkeer, het grensoverschrijdend zorggebruik en de intensieve veehouderij in deze regio. Samenwerking tussen verschillende disciplines zoals bio(techno)logische wetenschappen, geneeskunde (humaan én veterinair), nano- en microtechnologie en agrofood is belangrijk. Naast het versterken van het netwerk aan beide landsgrenzen, worden twee concrete tools verwezenlijkt. Enerzijds worden dankzij de ontwikkeling van het digitale meetinstrument Infectie Risico Scan (IRIS) infectierisico’s op objectieve en uniforme wijze in kaart gebracht. De IRIS meet processen zoals schoonmaak, handhygiëne door zorgverleners en juist gebruik van medische hulpmiddelen en antibiotica. Anderzijds is inzicht nodig in wie er waar, wanneer en waarom ziek wordt en op welke manier onderliggende ziekteverwekkers zich verspreiden. Softwarebedrijf Applied Maths zal met een aantal partners een Track en Trace Systeem (TTS) ontwikkelen en valideren in proeftuinen van zorg, publieke gezondheid en veehouderijen. Een complete a-tot-z procedure - van monsteropname tot interpretatie van de verzamelde data - wordt uitgewerkt zodat besmettingsroutes van antibioticaresistente ziekteverwekkers vroegtijdig opgespoord en aangepakt kunnen worden.

In 2014, the World Health Organisation declared antibiotic resistance to be one of the greatest threats to human health. Without supplementary measures, it is predicted that, by 2050, more people will die due to antibiotic-resistant pathogens than from cancer. Despite increasing resistance, however, no new antibiotics are expected to be marketed in the short term. i-4-1-Health, organised by the Amphia Hospital, with its main location in Breda, aims to reveal resistance amongst healthy citizens and patients and in the pork and poultry sectors. Professor Jan Kluytmans, who is heading the project, explains: Cross-border collaboration is crucial, as the spreading of antibiotic-resistant pathogens does not stop at national borders. This is specifically relevant in the Flanders-Netherlands border area, in view of the close trade and movement of persons, the cross-border use of healthcare facilities and the intensive cattle farming in the region. Collaboration between various disciplines, such as biological and biotech sciences, medicine (human and veterinary), nano and micro technology and agrofood is important. In addition to reinforcing the network on both national borders, two concrete tools have been created. On the one hand and thanks to the development of the digital measuring tool Infection Risk Scan (IRIS), infection risks can be analysed objectively and uniformly. IRIS measures processes such as cleaning, hand hygiene among healthcare providers and the correct use of medical resources and antibiotics. On the other hand, insight is required into who gets sick where, when and why, and into how the underlying pathogens are spread. With a number of partners, the software company Applied Maths will be developing and validating a track and trace system (TTS) in testing grounds for healthcare, public health and cattle farming. A complete A-Z procedure – from sampling to interpretation of the data gathered – is being set up to promptly trace and tackle the transmission routes of antibiotic-resistant pathogens.

Achievements (NL):

Bacteriën houden zich niet aan grenzen; niet van landen en niet van organisaties. Onder leiding van Amphia werkten 26 Vlaamse en Nederlandse organisaties de afgelopen drie jaar samen aan innovaties en onderzoek op het gebied van infectiepreventie en antimicrobiële resistentie in het Interreg project ‘i-4-1-Health’. De projectleiders, Prof. Jan Kluytmans (Amphia) en Prof. Herman Goossens (Universitair Ziekenhuis Antwerpen) vinden het i-4-1-Health project een bijzonder waardevol project. Het project heeft belangrijke informatie opgeleverd over het voorkomen van antibioticaresistentie in de grensregio. Het adagium van dit project was: “als je het niet kunt meten kun je het niet verbeteren”. Binnen 3 jaar is een eenduidig meetsysteem ontwikkeld en geïmplementeerd in ziekenhuizen, verpleeghuizen, kinderdagverblijven, scholen en veehouderijen. De samenwerking tussen de diergeneeskundige projectpartners en partners binnen de humane geneeskunde heeft geleid tot een beter begrip van de situatie en context omtrent antibioticagebruik en -resistentie tussen landen en domeinen. Het project heeft ons geleerd dat er naast overeenkomsten ook veel verschillen zijn tussen de landen en de verschillende deelnemende instellingen. Die verschillen bieden een basis om gericht te verbeteren. Het ontwikkelde meetsysteem heeft de situatie inzichtelijk gemaakt en door de metingen te herhalen wordt het effect van interventies inzichtelijk. Binnen het project is op deze manier al een zeer grote verbetering van de reinheid van de alle deelnemende ziekenhuizen bewerkstelligd. Dit project levert daarmee een unieke en belangrijke bijdrage aan de bestrijding van antibioticaresistentie in België en Nederland in de nabije toekomst. Op 11 oktober 2019 hebben projectpartners de resultaten gepresenteerd tijdens de slotbijeenkomst van het project in Breda. Highlights De projectdoelstelling is behaald: resistentie is inzichtelijk gemaakt bij gezonde burgers, patiënten en de (intensieve) varkens- en pluimveesector. Het project heeft hiertoe innovaties de Infectie RIsico Scan (IRIS) en een Track and Trace Systeem (TTS) voor resistente bacteriën ontwikkeld waarmee infectierisico’s op uniforme wijze in kaart zijn gebracht en waarmee inzicht is genereerd welke antibiotica resistente micro-organismen aanwezig zijn en hoe deze zich verspreiden. Binnen het i-4-1-Health project hebben ziekenhuizen, publieke gezondheidsdiensten (GGD'en en het Agentschap voor Zorg en Gezondheid), kennisinstellingen (Universiteit Utrecht, Universiteit Gent, Universiteit Antwerpen, de Gezondheidsdienst voor Dieren, UHasselt en Avans) en twee MKB bedrijven (CIDER en Applied Maths) samengewerkt door onderzoek te doen en (lokaal) beleid en protocollen te vergelijken voor de ontwikkeling en validatie van de IRIS. Bij drie projectpartners zijn laboratoriumruimtes verbeterd en opnieuw ingericht om de microbiologische projectactiviteiten uit te kunnen voeren, namelijk bij het Maastricht Universitair Medisch Centrum, het Universitair Ziekenhuis Antwerpen en de Universiteit Antwerpen. De projectpartners hebben samen met Applied Maths een track en trace systeem ontwikkeld. Deze projectpartners hebben intensief samengewerkt, methoden op elkaar afgestemd en onderzoek verricht, zodat in 3 verschillende centra het DNA van enkele resistente bacteriën op een vergelijkbare manier volgens eenzelfde methode in kaart kon worden gebracht. Dit maakt dat de data die middels de IRIS bij de pilootlocaties in de proeftuinen zorg, publieke gezondheidszorg en veehouderijen is verkregen met elkaar vergeleken kunnen worden. Via de ontwikkeling van de IRIS en het track en trace systeem zijn er dwarsverbanden gelegd tussen de proeftuinen Zorg, Publieke gezondheidszorg en Veehouderij en is discipline overstijgend onderzoek gestimuleerd.

Bacteria do not adhere to borders; not from countries and not from organisations. Under the leadership of Amphia, 26 Flemish and Dutch organisations have worked together over the past three years on innovations and research in the field of infection prevention and antimicrobial resistance in the Interreg project ‘i-4-1-Health’. The project leaders, Prof. Jan Kluytmans (Amphia) and Prof. Herman Goossens (University Hospital Antwerp) consider the i-4-1-Health project to be a particularly valuable project. The project has provided important information on the prevention of antibiotic resistance in the border region. The adage of this project was: if you can‘t measure it, you can’t improve it. Within 3 years, an unambiguous measurement system has been developed and implemented in hospitals, nursing homes, nurseries, schools and livestock farms. Cooperation between the veterinary project partners and partners in human medicine has led to a better understanding of the situation and context of antibiotic use and resistance between countries and domains. The project has taught us that in addition to similarities, there are many differences between the countries and the various participating institutions. These differences provide a basis for targeted improvement. The developed measurement system has made the situation clear and by repeating the measurements the effect of interventions becomes clear. In this way, the project has already achieved a very large improvement in the cleanliness of all participating hospitals. This project will make a unique and important contribution to the fight against antibiotic resistance in Belgium and the Netherlands in the near future. On 11 October 2019, project partners presented the results at the final meeting of the project in Breda. Highlights: The project objective has been achieved: resistance has been made clear to healthy citizens, patients and the (intensive) pig and poultry sector. To this end, the project has developed innovations in the Infection RIsico Scan (IRIS) and a Track and Trace System (TTS) for resistant bacteria with which infection risks have been identified in a uniform manner and which generates insight into which antibiotics resistant microorganisms are present and how they spread. Within the i-4-1-Health project, hospitals, public health services (GGDs and the Agency for Care and Health), knowledge institutions (University of Utrecht, Ghent University, University of Antwerp, the Health Service for Animals, UHasselt and Avans) and two SME companies (CIDER and Applied Maths) have worked together by conducting research and comparing (local) policies and protocols for the development and validation of the IRIS. Three project partners have improved and redesigned laboratory spaces to carry out the microbiological project activities, namely at the Maastricht University Medical Centre, the University Hospital Antwerp and the University of Antwerp. The project partners have developed a track and trace system together with Applied Maths. These project partners worked closely together, coordinated methods and carried out research so that the DNA of some resistant bacteria could be mapped in a similar way using the same method in three different centres. This makes it possible to compare the data obtained through the IRIS at the pilot locations in the test labs, public health care and livestock farms. Through the development of the IRIS and the track and trace system, cross links have been established between the healthcare, public health and livestock farms and discipline has been stimulated.
