La maladie du prolapsus pelvien (couramment appelé "descente d'organes") est causée par l’âge et les grossesses successives qui altèrent la santé de des femmes en endommageant les muscles pelviens, au niveau du bassin. Ce problème concerne une femme sur 3, soit plus de 2 millions de patientes uniquement en Belgique et en Hauts-de-France (60 % des femmes de plus de 60 ans). Bien que la demande de prise en charge chirurgicale soit croissante, les solutions actuelles sont mal adaptées, les dispositifs étant trop rigides et inconfortables pour les patientes. Le projet consiste en la réalisation d'implants textiles semi-résorbables de renfort pour le traitement de prolapsus génitaux. PROBIOMESH regroupe des acteurs transversaux et transfrontaliers dans le domaine des sciences médicales, polymères et procédés textiles pour proposer de nouvelles solutions aux patientes.

Pelvic prolapse disease (commonly referred to as “organ descent”) is caused by age and successive pregnancies that affect women's health by damaging pelvic muscles in the pelvis. This problem concerns one in three women, or more than 2 million patients in Belgium and the Hauts-de-France alone (60% of women over 60 years of age). Although the demand for surgery is increasing, current solutions are inadequate, as the devices are too rigid and uncomfortable for patients. The project consists of manufacturing semi-absorbable textile reinforcement implants for the treatment of genital prolapse. PROBIOMESH brings together cross-functional and cross-border stakeholders in the field of medical sciences, polymers and textile processes to offer new solutions to patients.

Prolaps, ook verzakking van de bekkenorganen genoemd, is een ziekte, veroorzaakt door de leeftijd en opeenvolgende zwangerschappen, die de gezondheid van de vrouw verstoren en de bekkenbodemspieren beschadigen. Dit probleem treft 1 op 3 vrouwen, hetzij meer dan 2 miljoen patiëntes alleen al in België en in Hauts-de-France (60% van de vrouwen van boven de 60). Hoewel de vraag naar chirurgische ingrepen toeneemt, zijn de huidige oplossingen weinig toereikend. De ringen zijn te hard en niet comfortabel voor de patiëntes. Het project bestaat uit de ontwikkeling van semi-resorbeerbare ondersteuningsimplantaten voor de behandeling van genitale prolaps. In PROBIOMESH werken transversale en grensoverschrijdende actoren op het gebied van de medische wetenschappen, polymeren en textielprocedés samen om de patiëntes nieuwe oplossingen voor te stellen.

Achievements (FR):

L’objectif de PROBIOMESH est de mettre au point des implants textiles de renfort les plus fins possibles, semi-résorbables, pour bouger mieux et plus longtemps. Pour cela, nous avons avons associé nos compétences : Centexbel (Courtrai et Grâce-Hollogne, Belgique) ainsi que l'Ecole Nationale Supérieure des Arts et Industries Textiles (ENSAIT) développent des solutions textiles à partir de nouveaux matériaux développés par Matéria Nova (Mons, Belgique). L'Ecole Centrale de Lille réalise les tests mécaniques et simulations nécessaires à l'amélioration des prototypes. Le CHU Lille coordonne le projet et apporte son expertise clinique sur la question. Les premiers travaux du projet ont permis d’arrêter un choix des matériaux sur lesquels nous travaillons, les polymères PLA et PCL. Ces 2 matériaux, non cytotoxiques, nous permettent d’obtenir des mélanges qui pourront ensuite être tricotés selon différentes techniques. Les premiers résultats de biorésorption et de comportement mécanique in vitro de ces deux matériaux sont également encourageants. Puisqu’il s’agit enfin de développer des implants davantage adaptés à la zone pelvienne, de tests pré-cliniques seront réalisés à partir des prototypes tricotés dans le cadre du projet Probiomesh. Ces derniers permettront d’évaluer le comportement des prototypes in vivo, à mettre au regard des résultats obtenus en in vitro. De nouveaux prototypes, à base des deux matériaux sélectionnés, ont été tricotés pour être ensuite étudiés in vitro et in vivo. Malgré la crise sanitaire liée à la pandémie de COVID-19, les partenaires oeuvrent toujours à la réalisation de premiers prototypes, conformes aux exigences médicales, à tester chez l’animal. En 2021, les partenaires ont réalisé de belles avancées dans la connaissance des matériaux et la compréhension des modes de dégradation et de l’évolution des propriétés physico-chimiques au cours du temps. La réalisation des tricots est, elle, plus délicate. Le caractère innovant et la technicité demandée pour le tricotage de ces matériaux innovants constituent encore un défi à relever en termes de qualité de réalisation et de reproductibilité. L’année 2021 s’est conclue par une avancée significative dans la résolution des problématiques de stabilité d’extrusion des filaments et de tricotage. Plusieurs prototypes très intéressants ont été obtenus et vont pouvoir faire l’objet de protocoles de tests in vitro avant d’envisager de procéder à une implantation in vivo. Le début d’année 2022 a permis de sélectionner les prototypes de mailles tricotées qui feront l’objet d’une évaluation in vivo au cours de l’année. De premiers résultats in vivo devraient être disponibles en toute fin d’année 2022 ou pour le début d’année 2023. Outre ces développements techniques, le partenariat Probiomesh a multiplié les participations à différents évènements scientifiques et médicaux, pour promouvoir les travaux de recherche sur de nouvelles meshes implantables et susciter l’intérêt des fabricants de dispositifs médicaux. Ces évènements sont annoncés sur le site web du projet : http://www.probiomesh-interreg.eu/ et sur les réseaux sociaux (LinkedIn, Twitter). Enfin, des articles ont été publiés dans des revues grand public (magazine Top Santé n°344 avril 2019, magazine Elle Belgique éd. Mai 2019) ou spécialisées (revue Unitex n°1 2019). L’objectif est de sensibiliser le grand public et les spécialistes aux travaux menés dans le cadre du projet, au bénéfice des patientes et de futures recherches.

The objective of PROBIOMESH is to develop the finest possible reinforcement textile implants, semi-resorbable, to move better and longer. For this, we have combined our skills: Centexbel (Courtrai and Grace-Hollogne, Belgium) and the Ecole Nationale Supérieure des Arts et Industries Textiles (ENSAIT) are developing textile solutions from new materials developed by Mattéria Nova (Mons, Belgium). The Ecole Centrale de Lille carries out the mechanical tests and simulations necessary for the improvement of prototypes. The CHU Lille coordinates the project and provides its clinical expertise on the issue. The first work of the project made it possible to make a choice of the materials we work on, the PLA and PCL polymers. These two materials, non-cytotoxic, allow us to obtain mixtures that can then be knitted according to different techniques. The first results of bioresorption and mechanical in vitro behaviour of these two materials are also encouraging. Since it is finally a question of developing implants more adapted to the pelvic zone, pre-clinical tests will be carried out from the prototypes knitted as part of the Probiomesh project. These will make it possible to evaluate the behaviour of prototypes in vivo, to be compared with the results obtained in vitro. New prototypes, based on the two selected materials, were knitted and then studied in vitro and in vivo. Despite the health crisis linked to the COVID-19 pandemic, the partners are still working on the first prototypes, meeting medical requirements, to be tested in animals. In 2021, the partners made great advances in the knowledge of materials and the understanding of the modes of degradation and the evolution of physico-chemical properties over time. The realisation of knits is more delicate. The innovative nature and technicality required for the knitting of these innovative materials is still a challenge in terms of quality of production and reproducibility. 2021 ended with a significant step forward in solving the problems of filament extrusion stability and knitting. Several very interesting prototypes have been obtained and will be able to undergo in vitro testing protocols before considering an in vivo implantation. At the beginning of 2022, the prototypes of knitted mesh were selected for in vivo evaluation during the year. First in vivo results should be available at the end of 2022 or by the beginning of 2023. In addition to these technical developments, the Probiomesh partnership has increased participation in various scientific and medical events, to promote research on new implantable meshes and to attract the interest of medical device manufacturers. Finally, articles have been published in general public magazines (magazine Top Santé n°344 April 2019, magazine Elle Belgique ed. May 2019) or specialised (Unityex magazine n°1 2019). Le consortium accompagnera l'essai clinique réalisé et le partenariat transfrontalier se poursuivra sur d'autres applications dans le futur.

Het project PROBIOMESH past in een onderzoeksdimensie en streeft naar een technologische doorbraak op het gebied van genitale prolaps. Het zal ook na het programma worden voortgezet. Het consortium zal de uitgevoerde klinische tests begeleiden en het grensoverschrijdende partnerschap zal in de toekomst aan andere toepassingen werken.

L'objectif est d'atteindre en milieu de programme deux entreprises dans le domaine du textile, des biomatériaux ou des implants. En fin de programme, le projet ambitionne, notamment à l'aide d'ateliers de valorisation, d'accompagner 5 entreprises du territoire transfrontalier dans l'introduction de nouveaux produits. D'autres partenariats de recherche sont attendus grâce à un effet de levier afin d'explorer toutes les possibilités de cette innovation de rupture. Grâce au réseau des partenaires, deux entreprises de plus sont ciblées, ce qui conduit à 7 participants au total. D'autre part, 90 implantations animales sont visées et permettront de valider les actions entreprises. Après 3 and de projet, les partenaires s'engagent à réaliser 4 prototypes différents, qui seront ensuite implantés in vivo et testés. L'objectif est fixé à 60 implantations.

Expected Outputs (EN):

The objective is to reach two companies at the centre point of the programme in the field of textiles, biomaterials or implants. At the end of the programme, the project aims, namely via development workshops, to support 5 companies in the cross-border territory with the introduction of new products. Further research partnerships are expected to be leveraged to explore all the possibilities of this breakthrough innovation. Thanks to the network of partners, two more companies are targeted, resulting in a total of 7 participants. In addition, 90 animal establishments are targeted and will make it possible to validate the actions undertaken. 3 years into the project, the partners commit to produce 4 different prototypes, which will then be implanted in vivo and tested. The objective is set at 60 implants.

Het doel is halverwege het programma twee bedrijven te bereiken in het domein van textiel, biomaterialen of implantaten. Op het eind van het programma heeft het project de ambitie om onder andere via valorisatieworkshops vijf ondernemingen uit de grensstreek te begeleiden bij het op de markt brengen van nieuwe producten. Er wordt verwacht dat er dankzij een hefboomeffect nog andere onderzoekspartnerschappen tot stand komen om alle mogelijkheden van deze baanbrekende innovatie te verkennen. Dankzij het partnernetwerk zijn twee bijkomende ondernemingen gevonden, wat het totaal op zeven deelnemers brengt. Daarnaast worden 90 implantaten bij dieren beoogd om de ondernomen acties te valideren. Na drie jaar in het project verbinden de partners zich ertoe vier verschillende prototypes te realiseren, die vervolgens in vivo zullen worden ingeplant en getest. Het streefcijfer bedraagt 60 implantaten.
