While Franco-Belgian cross-border cooperation in the field of health has made significant progress within the EU framework as regards access to quality local care, some areas remain unexplored. The COPROSEPAT project focuses on prevention and therapeutic patient education (TPE) while supporting a dynamic of health cooperation. It also aims to provide answers to the development of chronic diseases and, therefore, to deploy prevention and TPE actions with regard to cardiovascular diseases, diabetes, etc., based on cross-border health cooperation. On the basis of this intervention logic, the project partnership proposes to carry out a shared health diagnosis of the border territory; harmonise TPE practices in cooperation with health professionals; set up TPE actions to strengthen the power the chronically ill patient has over his or her health; strengthen access to care and TPE for the most vulnerable people; deepen the complementarity between the two sides of the border in terms of health and medico-social issues to integrate this new field of action.

De Frans-Belgische grensoverschrijdende gezondheidssamenwerking heeft in het kader van de EU voor opmerkelijke doorbraken gezorgd in de toegang tot kwalitatieve nabijheidsdiensten in de zorg. Sommige domeinen zijn evenwel nog niet aangepakt. Het project COPROSEPAT concentreert zich op de preventie en de therapeutische voorlichting van de patiënt (TVP) en ondersteunt tegelijkertijd een samenwerkingsdynamiek. Het wil bovendien antwoorden aanreiken op de ontwikkeling van chronische ziekten en dus preventie- en TVP-acties uitrollen voor cardiovasculaire ziekten, diabetes enz., steunend op de grensoverschrijdende gezondheidssamenwerking. Op basis van deze actielogica stelt het partnerschap van het project voor om een gedeelde gezondheidsdiagnose van de grensregio uit te voeren; de TVP-praktijken in samenwerking met de gezondheidswerkers te harmoniseren; TVP-acties uit te voeren om chronisch zieken meer controle over hun gezondheid te geven; de toegang tot de zorgverlening en de TVP van de zwakste personen te bevorderen; de complementariteit tussen de beide kanten van de grens op medisch en medisch-sociaal vlak uit te diepen met het oog op de integratie van dit nieuwe werkterrein.

Si la coopération transfrontalière franco-belge en matière de santé a, dans le cadre de l’UE, réalisé des avancées marquantes dans le domaine de l’accès aux soins de qualité de proximité, certains domaines restent inexplorés. Le projet COPROSEPAT se concentre sur la prévention et l’éducation thérapeutique du patient (ETP) tout en soutenant une dynamique de coopération sanitaire. Il vise en outre à apporter des réponses au développement des maladies chroniques et, donc, de déployer des actions de prévention et d’ETP à l’égard des maladies cardio-vasculaires, du diabète etc, en s’appuyant sur les coopérations transfrontalières sanitaires. Sur base de cette logique d’action, le partenariat du projet propose de réaliser un diagnostic santé partagé du territoire frontalier; harmoniser les pratiques en ETP en coopération avec les professionnels de santé ; mettre en place des actions ETP pour renforcer le pouvoir du patient malade chronique sur sa santé ; renforcer l’accès aux soins et à l’ETP des personnes les plus fragilisées ; approfondir la complémentarité entre les deux versants frontaliers sur le plan sanitaire et médico-social pour intégrer ce nouveau champ d’action.

Achievements (EN):

On December 31, 2022, we reached the end of the Interreg V FWVl COPROSEPAT project. The aim was to report on the work and achievements of the cross-border health cooperation project in the Franco-Belgian rural area developed during of its last years. Since 2008, patients from the Franco-Belgian border area have been able to receive treatment in hospital establishments located on either side of the border without administrative, legal or financial obstacles, thanks to ZOAST agreements (Organized Zones of Access to Cross-border Care) which cover the entire border territory. From April 1, 2019, Franco-Belgian cooperation has expanded to include the prevention and educational treatment of chronic pathologies, thanks to the project Interreg V France-Wallonia-Vlaanderen "COPROSEPAT" which brings together a large number of operators from the French and Belgian border slopes. Today, chronic conditions which are increasing, in part, following demographic aging constitute major challenges for health systems, particularly in rural border areas where the provision of care and health services is quite limited and/or far from the patient's usual place of life. Since its launch, the COPROSEPAT project has developed analysis work and set up various achievements. - Under the guidance of the Grand Est Regional Health Observatory (ORS GE), a health diagnosis of the cross-border territory was carried out, in the form of a “territory portrait” which highlights the particularities of the situation socio-economic of the project territory, the state of health of the population, the supply of care and health services available, etc. - One aspect of the project, led by the Health Observatory of the Province of Luxembourg, focused on improving access to care and therapeutic education systems (ETP) for vulnerable populations. A review of the literature relating to health promotion for vulnerable populations as well as directories of health prevention tools were carried out. Training to enable healthcare professionals to better support the target audience was provided in collaboration with CHRONILUX and the Belgian Cardiological League. - A module led by the Grand Est Regional Health Observatory (ORS GE) focused on Therapeutic Patient Education (ETP). A conceptual and comparative analysis of the ETP approach (financing, projects developed, etc.) in France and Belgium was carried out. Days of discussion and reflection on ETP were organized with different stakeholders (health professionals, associations, patients, health networks, etc.) with a view to seeking to harmonize practices and build a common culture. between the actors of the two border sides. But also boost the recognition of expert patients, or even ensure them the status of actor-advisor in health systems. 2 - An ETP course in heart failure was developed by Dr Laurence Gabriel of UCL Namur University Hospital. ETP training in heart failure for professionals has already been organized, and a major ETP brochure for heart failure patients and healthcare professionals has been developed and recently distributed to heart failure workshops. and public meetings with cross-border patients.

Expected Results (EN):

Inventory of the health situation in the cross-border area: assessment of the population affected by chronic pathologies, inventory of FTE services, mapping of the supply of cross-border care and services. More than 50,000 people are expected to benefit from support through the project. Organisation of two training sessions per year and 10 days of methodological support. Creation of a care unit for patients with heart failure.

Stand van zaken van de gezondheidszorg in de grenszone: evaluatie van de bevolking die aan chronische ziekten lijdt, inventaris van de TVP-diensten, in kaart brengen van het grensoverschrijdende zorg- en dienstenaanbod. Het project zou meer dan 50.000 personen ten goede komen. Organisatie van twee opleidingen per jaar en 10 dagen methodologische begeleiding. Oprichting van een zorgeenheid voor patiënten met hartfalen.

Expected Results (FR):

Etat des lieux de la situation sanitaire de l'espace transfrontalier : évaluation de la population atteinte de pathologies chroniques, inventaire des services en ETP, cartographie de l'offre de soins et de services transfrontaliers. Plus de 50 000 personnes devraient bénéficier d'un soutien par le projet. Organisation de deux formations par an et de 10 jours d'accompagnement méthodologiques. Création d'une unité de soins pour les patients insuffisants cardiaques.

Structuring a network of the cross-border space around patients in order to act on their environment and to coordinate educational, health and therapeutic action in this direction. Carrying out a shared health diagnosis of the cross-border area and recognition of patient education as a therapeutic process. Strengthening access to care: detection of precarious situations, creation of a care unit for patients with heart failure.

Structurering van een grensoverschrijdend netwerk voor de patiënten, om in te spelen op hun omgeving en de educatieve, gezondheidsgerichte en therapeutische acties in die zin te oriënteren. Uitvoering van een gedeelde gezondheidsdiagnose van de grenszone en erkenning van de opvoeding van de patiënt als therapeutisch proces. Verbetering van de toegang tot zorgverlening: opsporen van precaire situaties, oprichting van een zorgeenheid voor patiënten met hartfalen.

Expected Outputs (FR):

Structuration d'un réseau de l'espace transfrontalier autour des patients en vue d'agir sur leur environnement et d'articuler l'action éducative, sanitaire et thérapeutique en ce sens. Réalisation d'un diagnostic santé partagé de l'espace transfrontalier et reconnaissance de l'éducation du patient comme processus thérapeutique. Renforcement de l'accès aux soins : dépistage de situations précaires, création d'une unité de soins pour les patients insuffisants cardiaques.
