Description (EN): "This project is part of the plan PRO SOL": The project intends to meet the needs of services required for the support of elderly people at home in rural and/or peripheral areas. The goals are to slow down the loss of independence of the seniors and enable them to stay at home the longest possible, to reduce inequalities in access to health services and to improve the patients’ care pathway.
The reorganization of health and social services requires an Italian-French strategic vision in terms of development of the territorial ecosystem (prevention capacities, support for mountain communities and sustainability).
This plan foresees 4 projects defined in relation to the target groups: young people, women, elderly people and proximity. The objectives are socio-health integration and the development of open and united communities, with new work opportunities in innovative services, with a view to supporting families in areas that are not easily accessible.
For each project, a working method is planned with common phases, the study of the environment (until 2022), the qualitative and quantitative monitoring of the resources and services in place, through assessment assessments by users, professionals. and the socio-sanitary operators,
The selection of appropriate instruments to increase services in the community, with a jointly planned feasibility study. Experimenting with innovative solutions, benchmarking results and preparing guidelines for defining common policies. The Proximity project will allow constant exchange between public organizations and the social private sector for the production of common tools for analysis, implementation and programming, and the experimentation, within each target project, of new models of perennial proximity welfare.
Read more Description (FR): "Ce projet fait partie du plan PRO SOL": Le projet entend répondre aux besoins de services nécessaires au maintien des personnes âgées à domicile dans des zones rurales et/ou périphériques. Les objectifs sont de ralentir la perte d’autonomie et permettre aux seniors de rester au domicile le mieux et le plus longtemps possible, réduire les inégalités d’accès aux services de santé et améliorer le parcours de soins du patient.
Les réorganisations des services sanitaires et sociaux nécessitent une vision stratégique italo-française en matière de développement de l’écosystème territorial (capacités de prévention, de soutien aux communautés de montagne et durabilité).
Ce plan prévoit 4 projets définis compte tenu des groupes-cibles : jeunes, femmes, personnes âgées et proximité. Les objectifs sont l’intégration socio-sanitaire et le développement de communautés ouvertes et solidaires, avec nouvelles opportunités de travail dans des services innovants, en vue du soutien aux familles dans les zones pas facilement accessibles.
Pour chaque projet, est prévue une méthode de travail avec des phases communes l’étude du milieu (jusqu’en 2022), le suivi quali-quantitatif des ressources et des services en place, à travers des évaluations patages par les usagers, les professionnels et les opérateurs socio-sanitaire,
La sélection d’instruments appropriés en vue de l’augmentation de services dans la communauté, avec une étude de faisabilité projetée conjointement. L’expérimentation de solutions innovantes, l’évaluation comparative des résultats et la préparation de grandes lignes pour la définition de politiques communes. Le projet Proximité permettra l’échange constant entre les organisations publiques et le privé social pour la production d’outils communs pour l’analyse, la réalisation et la programmation, et l’expérimentation, au sein de chaque projet target, de nouveaux modèles de welfare de proximité pérennisables.
Read more Description (IT): "Questo progetto fa parte dell'piano PRO SOL": Il progetto intende rispondere al bisogno di servizi necessari per il mantenimento delle persone anziane a domicilio nelle zone rurali e/o periferiche. Gli obiettivi sono di rallentare il più possibile la perdita di autonomia e permettere un invecchiamento attivo che permetta agli anziani di restare a casa il più lungo possibile, ridurre le disparità nell'accesso ai servizi sanitari et migliorare il percorso di cura del paziente.
La riorganizzazione dei servizi sanitari e sociali richiede una visione strategica italo-francese per lo sviluppo dell'ecosistema territoriale (capacità di prevenzione, sostegno alle comunità montane e sostenibilità).
Questo piano prevede 4 progetti definiti in relazione ai gruppi target: giovani, donne, anziani e prossimità. Gli obiettivi sono l'integrazione socio-sanitaria e lo sviluppo di comunità aperte e solidali, con nuove opportunità di lavorare in servizi innovativi, per supportare le famiglie in aree non facilmente accessibili.
Per ogni progetto esiste un metodo di lavoro con fasi comuni lo studio dell'ambiente (fino al 2022), il monitoraggio qualitativo-quantitativo delle risorse e dei servizi in atto, attraverso patch di valutazione da parte di utenti, professionisti e gli operatori socio-sanitari,
La selezione di strumenti adeguati per aumentare i servizi nella comunità, con uno studio di fattibilità pianificato congiuntamente. Sperimentare soluzioni innovative, confrontare i risultati e preparare linee guida per la definizione di politiche comuni. Il progetto Proximity consentirà lo scambio costante tra le organizzazioni pubbliche e il settore sociale privato per la produzione di strumenti comuni per l'analisi, la realizzazione e la programmazione e la sperimentazione, all'interno di ciascun progetto target, di nuovi modelli di benessere perenne per prossimità.

Pro.Sol-Senior in Aosta Valley is committed to offering new rehabilitation pathways to elderly people with neurodegenerative diseases. During the course from the project, getting more and more in touch with the unexpressed needs of people, the journey has led to experimenting not only a way of taking in charge, but above all a method, an approach to preventing the pathologies and well-being of people and their families. Following this path, Pro.Sol-Senior has concluded with the event “Interferenze. Short-circuit places where culture ‘cares’”, which has resulted in initiatives of cultural well-being open to all, i.e. the integration of culture and health for the well-being of people. The idea was that of a generalised event, even in unconventional places in relation to the theme of health. Interferenze has “occupied” 11 places, the theatre of the youth citadel, the episcopal seminary, the room of the regional library, the large hall of the University of Val d’Aosta, the Place Arco d’Augusto and Place Emile Chanoux, the Museum of Contemporary Art Castello Gamba, the Cultural Centre of Villa Michetti in Pont-Saint-Martin, the Family Centre of Aosta, Castello di Issogne and the forensic cryptoportique d’Aosta. Interferenze brought people together. More than 700 people participated in the proposed events, to listen, speak and get to know more than 20 guests, very different from each other, also in this case to rethink the maps and enrich the knowledge on the theme of active aging and dementia from different perspectives of people who, in different ways, face them.
