Description (EN):

The so-called "rare diseases" do not receive adequate study and research attention from the scientific community and pharmaceutical industry. Many of these diseases occur in children and require continuous diagnostic and therapeutic efforts to improve the quality of life of affected children and to minimize the socio-economic impact on families and health system. In the Programme Area already exist consolidated clinical and scientific skills, especially in the cross-border axis between the IRCCS Burlo Garofolo of Trieste and the Pediatric clinic of the University of Ljubljana who already cooperate on research on some rare diseases such as immune-mediated inflammatory diseases. However, the following are still missing: a) Patient-centered diagnostic methods capable of defining specific individual pathological profiles and targeted treatments. b) A widespread knowledge throughout the territory on diagnostic and therapeutic possibilities in this sector. c) Efficient tools for knowledge /research results sharing, especially towards general practitioners, hospital doctors and patient associations. The project CATTEDRA intends to address these shortcomings by bringing together clinical centers, research centers and companies.

Le cosiddette “malattie rare” non ricevono adeguata attenzione di studio e ricerca da parte della comunità scientifica e dell’industria del farmaco. Molte di queste malattie si manifestano in età pediatrica e richiedono continui sforzi diagnostici e terapeutici per migliorare la qualità di vita dei bambini affetti e per minimizzare l’impatto socio economico sulle famiglie e sul sistema sanitario. Nell’Area Programma esistono consolidate competenze cliniche e scientifiche, in particolare nell’asse transfrontaliero IRCCS Burlo Garofolo di Trieste e Clinica pediatrica dell’Università di Lubiana che già collaborano sulla ricerca nell’ambito di alcune specifiche malattie rare: le patologie infiammatorie immuno-mediate. Queste, tuttavia, mancano ancora di: a) Metodologie diagnostiche centrate sul paziente in grado di definire lo specifico profilo patologico e selezionare trattamenti mirati. b) Conoscenze diffuse sul territorio sulle possibilità diagnostiche e terapeutiche in quest’ambito clinico. c) Efficienti strumenti di condivisione delle conoscenze e dei risultati della ricerca, soprattutto verso i medici di base, gli altri medici ospedalieri e le associazioni di pazienti. Il progetto CATTEDRA intende affrontare tali carenze riunendo Centri clinici, Centri di Ricerca ed Imprese.

Znanstvena stroka in farmacevtska industrija ne posvečata ustrezne pozornosti t.i. “redkim boleznim” v smislu študij in raziskav. Številne od teh bolezni, ki se pojavljajo v otroškem obdobju, zahtevajo veliko diagnostičnega in terapevtskega truda za izboljšanje kakovosti življenja obolelih otrok in zmanjšanje socialno-ekonomskih učinkov na njihove družine in zdravstveni sistem. Na Programskem območju obstajajo dolgoletne klinične in znanstvene izkušnje, predvsem tiste na čezmejni ravni, med tržaško otroško bolnišnico IRCCS Burlo Garofolo in Pediatrično kliniko Univerzitetnega kliničnega centra v Ljubljani, ki že sodelujeta pri raziskovanju nekaterih posebnih redkih bolezni: To so imunsko pogojene vnetne bolezni. Kljub temu obstajajo številne pomanjkljivosti, in sicer: a) Diagnostične metode, usmerjene na bolnika, ki bi omogočile opredelitev natančne bolezenske slike pacienta in izbiro prilagojenega zdravljenja. b) Širše poznavanje diagnostičnih in terapevtskih možnosti na tem kliničnem področju na lokalni ravni. c) Učinkovita orodja za izmenjavo znanja in rezultatov raziskav, predvsem med splošnimi zdravniki, drugimi zdravniki v bolnišnici in društvi bolnikov. Z vključitvijo kliničnih centrov, raziskovalnih centrov in podjetij želi projekt CATTEDRA zapolniti te vrzeli.

Achievements (EN):

The general objective of the project is to bring together, share and improve the therapeutic methodologies and diagnostic possibilities of rare diseases in children in the area of paediatric rheumatology. The project will strengthen collaboration between the two hospitals to increase knowledge surrounding juvenile autoimmune diseases in neighbouring regions. Better has set up four different clinical registers in one application. When clinical data is entered, the complete clinical data database is obtained. In the first phase, two registers for juvenile idiopathic arthritis (JIA) and juvenile dermatomyositis (JDM) created. This involved the digitization of 23 forms. The registers have been active since August 2021. The second and third phases were completed in March 2022. The second phase included the creation of digital registers for two other diseases: systemic lupus erythematosus (SLE) and autoinflammatory disease (AID). The two registers involved the digitization of 9 different forms. Better has also developed add-on modules that are unrelated to specific registers and include lab results, Covid-19 investigations, and medications. In the third phase, Better created graphical diagrams of the patients’ clinical data for all 4 registers. Better's solution translates into the collection, management and use of data for two purposes: To support active patient monitoring and disease management to improve children’s health and overall quality of life. To become a research tool for daily clinical practice to used in conjunction with the clinical information system.

Expected Results (EN):

The overall project objective is to bring together, share and improve the therapeutic methodologies and diagnostic possibilities of rare diseases in children. At the time being, the two clinical reference centers of the pediatric age are working independently. It is intended to transfer specialist knowledge from clinical and research centers to important companies in the "Health" sector, in the Programme Area.

Expected Results (IT):

L’obiettivo complessivo del progetto è riunire, integrare, condividere e migliorare le metodologie terapeutiche e le possibilità diagnostiche delle malattie rare in età pediatrica. In questo momento esser risultano essere sviluppate in maniera autonoma dai due centri clinici di riferimento dell’età pediatrica. S’intende trasferire e condividere conoscenze specialistiche da centri clinici e di ricerca verso le imprese del settore “Health”, di fondamentale importanza nell’Area di Programma.

Splošni cilj projekta je združiti, nadgraditi, izmenjati in izboljšati terapevtske metode in možnosti diagnostike redkih otroških bolezni, ki jih trenutno vsaka referenčna ustanova v pediatriji razvija samostojno. Projekt želi zagotoviti izmenjavo in prenos strokovnega znanja iz klinično-raziskovalnih centrov na podjetja, ki delujejo na področju zdravstva, kar je za programsko območje ključnega pomena.

Expected Outputs (EN):

Two are the results of the project. The first is the transfer, integration and sharing of diagnostic and therapeutic data on therapies. To achieve this result, cooperation between "scientific" partners and "business" partners is foreseen in line with output indicator for IP 1.b. This result will not only strengthen cooperation between two key pediatric bodies but will also provide data and guidelines on common therapeutic protocols useful to hospitals/doctors/healthcare professionals involved with children affected by immune-mediated diseases. The second result is a feasibility study of 4 innovative diagnostic tests developed putting together the experiences of Burlo and UCKLJ altogether with the technical support by CO BIK. Also for this result, close collaboration public / private partners is foreseen to transfer useful knowledge from clinical /research centers to companies, contributing to the programme output indicator CO26. The feasibility study of new shared diagnostic tests will allow to lay the basis for new cross-border collaborations but also to stimulate Italian and Slovene companies in the biomedical sector to cooperate with research bodies, in particular within the diagnostic sector in vitro.

Expected Outputs (IT):

I risultati del progetto sono due. Il primo è il trasferimento, l’integrazione e condivisione di dati in ambito diagnostico e terapeutico sulle terapie. Per raggiungere questo risultato è prevista una cooperazione tra i partner “scientifici” ed i partner “aziende” come previsto dall’indicatore di output specifico per la PI 1.b. Questo risultato non solo rafforzerà la cooperazione tra due enti chiave della pediatria ma anche fornirà dati e linee guida su protocolli terapeutici comuni utili agli ospedali, ai medici e agli operatori sanitari dell’Area di Programma che si trovano a dover affrontare i problemi legati a bambini affetti dalle malattie immuno-mediate. Il secondo risultato è fare uno studio di fattibilità di 4 innovativi test diagnostici sviluppati mettendo assieme le esperienze sviluppate dal Burlo e UCKLJ e le competenze tecniche di CO BIK. Anche per questo risultato del progetto è prevista una stretta collaborazione tra partner pubblici e partner privati al fine di trasferire conoscenze utili dai centri clinici e di ricerca alle imprese, contribuendo al raggiungimento dell’indicatore di output del programma CO26. Lo studio di fattibilità di nuovi test diagnostici condivisi consentirà di porre le basi per nuove possibili collaborazioni transfrontaliere ma anche di stimolare altre imprese del settore biomedicale, sia italiano che sloveno, ad attivarsi per collaborazioni con enti di ricerca, in particolare nel settore della diagnostica in vitro.

Projekt bo imel dva končna rezultata: prvi je prenos, nadgraditev in izmenjava podatkov o diagnostičnih in terapevtskih postopkih. K doseganju tega rezultata bo pripomoglo sodelovanje med »znanstvenimi« partnerji in partnerji podjetniške sfere, kot to predvideva specifični kazalnik učinka za PN 1.b. Ta rezultat ne bo samo okrepil sodelovanja med ključnima pediatričnima ustanovama, temveč bo ponudil tudi podatke in smernice o skupnih terapevtskih protokolih, koristnih za bolnišnice, zdravnike in zdravstvene delavce s programskega področja, ki se soočajo s težavami otrok z imunsko pogojenimi boleznimi. Drugi končni rezultat je študija izvedljivosti štirih inovatih diagnostičnih testov, ki bodo razviti ob združevanju izkušenj bolniščnic Burlo in UCKLJ s tehničnimi kompetencami CO BIK. Tudi pri tem rezultatu bodo javni in zasebni partnerji tesno sodelovali z namenom prenosa koristnih znanj iz kliničnih in raziskovalnih centrov na podjetja, kar bo prispevalo k doseganju kazalnika učinka za program CO26. Študija izvedljivosti novih skupnih diagnostičnih testov bo služila kot osnova za nova bodoča čezmejna sodelovanja in bo lahko spodbudila druga italijanska in slovenska podjetja s področja biomedicine k vzpostavitvi sodelovanja z raziskovalnimi ustanovami, predvsem na podorčju in vitro diagnostike.
