Stroke is a leading cause of death or disability in adults in the program region. A fast response and diagnosis is key to ensure the best possible treatment outcome as every minute costs valuable brain capacity and decreases chances for an independent life after a stroke. There are several challenges, such as long distances between patient and hospitals and cooperation between e.g. hospitals that may prolong the time from stroke to treatment. This network project is a collaboration between Zealand University Hospital and the University Hospital Schleswig-Holstein and will analyze the possibility of a Mobile Stroke Unit (MSU): An ambulance equipped with a CT-scanner to diagnose patients in the ambulance and start treatment faster.

Schlaganfall ist eine der Hauptursachen für Tod oder Behinderung bei Erwachsenen in der Programmregion. Ein schneller und nahtloser Respons ist der Schlüssel zur Sicherstellung des bestmöglichen Behandlungsergebnisses, da jede Verzögerung wertvolle Gehirnkapazität kostet und die Chancen auf ein selbständiges Leben nach dem Schlaganfall verringert. Es gibt verschiedene Herausforderungen, wie z. B. große Entfernungen zwischen Patient und Krankenhaus und die Zusammenarbeit zwischen bspw. Krankenhäusern, die die Zeit vom Schlaganfall bis zur Behandlung verlängern können. Dieses Netzwerkprojekt ist eine Kooperation zwischen dem Universitätsklinikum Seeland und dem Universitätsklinikum Schleswig-Holstein und wird die Möglichkeit einer Mobile Stroke Unit (MSU) analysieren: Ein Krankenwagen, ausgestattet mit einem CT-Scanner um Patienten im Krankenwagen zu diagnostizieren und schneller zu behandeln.

Slagtilfælde er en af de førende årsager til død eller handicap hos voksne i programregionen. En hurtig og sammenhængende respons er afgørende for at sikre det bedst mulige behandlingsresultat, da hver forsinkelse koster værdifuld hjernekapacitet og reducerer chancerne for et selvstændigt liv efter slagtilfældet. Der er flere udfordringer, såsom lang afstand mellem patient og hospital og samarbejde mellem fx hospitaler, der kan forlænge tiden fra slagtilfælde til behandling. Dette netværksprojekt er et samarbejde mellem Sjællands Universitetshospital og Schleswig-Holsteins Universitetshospital og søger at møde disse udfordringer ved at analysere muligheden for en Mobile Stroke Unit (MSU): En ambulance udrustet med en CT-scanner for at diagnosticere i ambulancen og starte behandlingen hurtigere.

Achievements (EN):

A stroke (i.e.: stroke) always occurs suddenly and is one of the most time-critical diseases and, in addition, a common disease: in Denmark alone (5.8 million inhabitants) there are 12.000 new strokes per year. Early care is essential, u, after a stroke, to lead a self-determined life or even survive, because every minute that passes means a loss of brain function. After only 4½ hours, treatment options are limited. In much of the region of Zealand and northern Germany, people live quite scattered with greater distances to specialised and time-critical supply. The project has therefore launched a new cooperation between the University Hospital Seeland and the University Hospital Schleswig-Holstein to examine how to develop a Mobile Stroke Unit (MSU) in the Seeland and Lübeck region. An MSU is a ready-made ambulance, especially equipped with a scanner, whereby a diagnosis can be made in the vehicle and the fastest possible treatment can be initiated. This preserves valuable brain capacities and increases the chances of an independent life after stroke. As part of the network project, it was examined how the availability of supply services can be significantly improved. Through this cross-border cooperation, the project partners were able to share their knowledge in order to develop and model a new method for the faster treatment of strokes. RESULTS: • Recording the possibilities of citizens to treat strokes in the regions of Seeland and Lübeck. • Gain experience from ongoing projects on mobile scanners. • Technical knowledge of mobile scanners and the organizational structure. • Knowledge exchange with a company about requirements and technical data. • Model simulation with significant time savings compared to data from previous processes.

Expected Results (EN):

The goal is to improve the health of the citizens in the program region by improving and increasing acute stroke treatment. To reach the overall goal, this project will establish a collaboration between Zealand University Hospital, Roskilde and the University Hospital Schleswig-Holstein, Lübeck and analyze the possibility of implementing a Mobile Stroke Unit (MSU). The project will both clarify and identify barriers and opportunities for setting up MSUs and show the pathway for a successful set-up in both Region Zealand and Schleswig-Holstein. To secure a good process and to be able to share the results of the project with the rest of the program region we have secured the Interreg Deutschland-Danmark project “Access Acceleration” as a network partner (NP1). Furthermore, “Interreg112” and “Saving Life” have expressed interest in participating as network partners. This is important for the project in regards to knowledge on acute care and innovation of complex health care. Later, others may be invited to build a strong main project. To build a MSU set-up will demand collaboration between professionals across the prehospital and hospital sectors as well as collaboration between public and private health care providers like e.g. health insurance companies and public and private health contractors (e.g. Falck in DK, Krankenkassen in DE). The cross border collaboration and network project form the basis for a larger project about a faster stroke response in the program region. The results of the analysis in the network project will form the basis for a larger project about innovative use of CT-scanners in ambulances supporting and improving the existing health care infrastructure in the program region. The network project will entail mapping of current workflows, contacting possible new partners, suggesting model simulations and creating a platform for the exchange of ideas. This will lead to the development of a main project based on the attained knowledge. It is the ambition that a successful network project will lead to the implementation and integration of a MSU through a main project.

Expected Outputs (EN): One work flow chart of the Danish and German patient pathway of a stroke. Collected information for all project partners; One set of collected information for all project partners; 2-3 suggestions for model simulations; One cross border workshop is held; One project description is drafted.
