Rehabilitation has an important role in improving the health and well-being of the population and the overall efficiency and sustainability of the health systems. The ageing of the population will increase the number of persons who need rehabilitation, while there is a shortage of healthcare staff. Currently, access to rehabilitation is uneven both between and within countries. Technology offers the possibility to provide efficient care and access to dedicated rehabilitation care regardless of the location of a patient. Different forms of rehabilitation therapy using virtual technologies are already taking place. However, these digital applications merely are used in a local setting and economies of scale are not exploited. Transnational cooperation is needed to reach these economies of scale, bringing the digital transformation of the rehabilitation sector to a next level. In order to ensure equal access to rehabilitation care and foster the resilience of health systems, this project will make an important step forwards in the digitalization of the rehabilitation sector, being the first in building an open NWE virtual rehabilitation ‘clinic’, through which existing virtual rehabilitation therapies will be scaled up. 8 regional pilots (NL/DE/IR/LU), which act as showcases, will be implemented in which rehabilitation professionals in various healthcare settings integrate virtual rehabilitation in their daily practice. Additional, capacity building activities will be carried out. The main outputs: -A running open virtual rehabilitation clinic including a minimum of 12 virtual rehabilitation interventions -8 finalized pilots in which rehabilitation practitioners have used the virtual rehabilitation clinic to integrate several virtual rehabilitation interventions in their daily practice and have reached a minimum of 280 patients in their rehabilitation at home - A package of capacity building activities for patients, healthcare practitioners and enterprises.

Actual Achievements (EN):

Kick-off: During the XR4REHAB conference on June 24, 2023 in Amsterdam, the public kick-off of the project occurred with a plenary Scale-Up4Rehab session, initiating discussions with stakeholders, including SMEs and startups in the digital health sector, regarding the future Virtual Rehabilitation Clinic's impact on identified target groups. The official kick-off took place on September 1st at the Sint Maartenskliniek, where representatives of Interreg, RVO, and all consortium partners were present and the contract was officially signed. Project Management: An annual plan has been devised, featuring monthly online project meetings and four online Ideation Sessions to outline the Virtual Clinic's development and requirements. Consortium partners established project teams, initiated internal discussions, and a project management team (PMT) was formed. Dependencies between activities are being identified, and the writing of a quality & mitigation plan is in progress. WP1: Preceding the official kick-off meeting, project partners met at the Sint Maartenskliniek on August 31, 2023, for a project meeting and a guided brainstorming session. During the session partners were led to a solution to have elderly people beg for digital healthcare. (file NWE0100082-Scaleup4rehab – brainstorm20230831) The following Ideation Sessions, during which contributions were made to the framework development of the virtual rehabilitation clinic, were held online: Ideation Session 1 was held on 26 September 2023 with the aim to discover what digital platforms already exist, guest speaker Herko Coomans from the Ministry of Health, Welfare and Sport, provided an overview of European Digital Health regulations – organized by LP1 (file NWE0100082-Scaleup4rehab – Ideation1) Ideation Session 2 was held on 26 October 2023 and aimed to have a closer look at the involved stakeholders - organized by PP5 and PP7 (file NWE0100082-Scaleup4rehab – Ideation2) Ideation Session 3 was held on 5 December 2023 and involved investigating and collecting expert opinions from across the consortium regarding the criteria that will be included in the approval process for the acceptance of digital health interventions on the virtual rehabilitation clinic – organized by PP4 and PP8 (file NWE0100082-Scaleup4rehab – Ideation3) In addition, PP4/PP8 have worked on a scoping review of the literature investigating existing frameworks that evaluate digital health applications, which is now in the data extraction phase. Most partners have engaged with different types of stakeholders in the various countries investigating platform preferences, values of VR, mastery of VR, and involvement in development. The set-up of stakeholder panels has been initiated, but they will be made effective in period 2. PP9 started a structured literature review on the type of market (i.e. 3-sided market) in which the clinic would operate with the goal to identify where barriers for adoption and scaling could reside for the different types of players on this market. Based on the first results of the literature review different approaches to the business model have been outlined with different approaches to scale the VR-applications being proposed, as well as different ‘manipulations’ with regard to parameters of the Virtual Clinic’s business model that will be included in the pilot experiments. All work done in period 1 will be part of the process leading to the Functionality and Design report of the digital infrastructure that is going to be build. WP2: In preparation for pilot studies in Ireland, UCC/UCD scheduled focus groups with clinical physiotherapists to explore barriers and facilitators to implementing virtual reality interventions. Ethical approval has been prepared, and collaboration with physiotherapy schools is underway. WP3: UCC advanced WP3 commencement, assigning tasks for creating a VR module in the physiotherapy curriculum. A dependencies document was created to coordinate activities across work packages, and progress has begun on a User Manual. Communication: The communication group, led by PP6 Health Valley and PP10 Eurasanté, developed a strategy, plan, and action plan for communication during RP2. A Teams environment facilitates collaboration, and promotional materials such as posters and flyers have been designed. Presentations and publications stemming from the project are ongoing. An in-person Spring Meeting was organized in Brussels at 14 and 15 March 2024, where all consortium partners came together to discuss the project progress. The Platform brief was presented and approved. The Platform Brief forms the basis for all other activities in the project. It outlines the vision, mission and strategy, but also gives the insights in the guiding principles, several treatment scenarios and the key-users needs and barriers. It also provides a draft of what the Virtual Rehabilitation Clinic can be by describing the ‘Why’, the ‘What’, and a draft of the ‘How’. WP1 The ‘Functionality and design report’ for the Virtual Rehabilitation Clinic is finalized. The document is the result of three phases: - Phase 1: 4 Ideation sessions; - Phase 2: Research, writing and presenting the Platform Brief; - Phase 3: Based on the Platform Brief the Functional and Technical Requirements document is written, what is the basis for the tender process in search of a digital platform builder. LP1 worked closely together on the document with PP2 and PP4. A Request for Information and Request for Proposal was created as an extract of the Functionality and Design report and was sent out to 5 digital companies. A selection committee is formed to score the offers that will be received in Period 3. PP9 conducted a structured literature review on the type of market (i.e. 3-sided market) that the virtual clinic would operate in with the goal to identify where barriers to adoption and scaling could reside for the different types of players on this market. The review is currently in the write-up phase and is expected to be finalized by the end of 2024. WP2 The prepilots of PP5 and PP10 will start in period 3 and preparations are in full swing. As it is expected that the virtual clinic will not have a prototype ready to be tested in period 3, a simple webshop has been created (the Soapshop), which could be used for testing. All partners are preparing their pilots to be started in period 3 or 4. WP3 Brainstorm sessions have been held to discuss the delivery of the various WP3 deliverables. The Functionality and Design Report for the Virtual Coaching Centre was drafted by LP1, along with the input of the other work package partners. Communication PP6 and LP1 produced a marketing video explaining the project from the perspective of the different stakeholders. The Scale-Up4Rehab session at the World Congress on Virtual Rehabilitation was an excellent opportunity to present the video to a large group of stakeholders and to discuss the project plans with them. Many consortium partners joined the congress and WP-specific communication plans are created by PP6 and PP10 to fulfill the communication needs of each work package and support their implementation. Project management Monthly videoconference meetings took place with updates on project management and Work Packages. LP1 drafted the agenda’s, chairmanship alternated. A physical Autumn Meeting was organized by LP1 on 14-15 November. During the meeting, which was held in the Four Elements Hotel in Amsterdam, all partners shared their progress and several workshops were held. The selected ICT company, Arlanet, presented themselves and they had the partners reflect on the brand identity of the virtual clinic and the needs of the different target groups through various exercises. WP1 Monthly meetings were held with PP6 and PP9. LP1 led the tender procedure and selection of the ICT company that will build the virtual clinic. In order to manage the tender process, external expertise was hired. Of the 5 companies that were asked to submit a quote, actually a quote from 2 companies was received. The selection committee consisted of employees from LP1, PP2, PP4 and PP6. After a scoring procedure including a company presentation and a round of questions and answers a partner was selected: Arlanet. The contracting was done by LP1 on behalf of the consortium, resulting in a first contract to build a Minimal Viable Product (to be finished in Period 4). The following activities were undertaken together with the supplier in period 3: a Kick-off meeting, Customer Journey Sessions, Consortium Workshop during the autumn meeting, User Research, Co-creation sessions, a Branding Workshop and a technical architecture meeting. All partners were involved. After the meetings online feedback was given to the entire consortium. PP4 and PP8 have finalised and circulated a final set of core criteria to assess the eligibility of online interventions for the virtual clinic. They have also drafted a Terms of Reference document and circulated said document to the consortium in order to establish the expert committee (D 1.2.2). PP4 and PP8 have submitted a scoping review titled "A Scoping Review of Frameworks Evaluating Digital Health Applications". This scoping review has been submitted to the journal of Digital Health, wherein they have completed two rounds of revisions to the manuscript, and are currently awaiting to hear a final decision regarding plans for publication in Digital Health. PP9 finished a structured literature review on the type of market (i.e. 3-sided market) that the virtual clinic would operate in with the goal to identify where barriers to adoption and scaling could reside for the different types of players on this market. WP2 Regular meetings between several partners to align the development of the Virtual Rehabilitation Clinic with the regional pilots, business plan and other interdependencies. LP1 discussed and aligned the planning of the prototype and the MVP with PP5 and PP10, in order to improve the development of the Virtual Rehabilitation Clinic with the feedback of the pre-pilots. PP4 held their first workshop in October 2024 in Diepenbeek, Belgium, using the PERsona-CEntered Participatory Technology (PERCEPT) co-creation method. All partners have recorded their plans for the pilots in the pilot design document. Most partners already started their pilots evaluating various digital applications. WP3 A number of meetings with WP3 activity leads were held to coordinate on activity deliverables. PP4 drafted three separate User Manuals specifically for students, patients, and practitioners. Two live webinars were organized by associate partner XR4REHAB to educate practitioners about various use cases of XR rehabilitation as part of capacity building efforts for digital rehabilitation. LP1 and PP4 were involved in the organization. LP1 and PP4 brainstormed about the online training modules for the different user groups of the virtual clinic. PP8 carried out research which focused on the integration of virtual reality (VR) technology into the physiotherapy programme at University College Dublin. This research is key to embedding virtual reality for rehabilitation into physiotherapy curriculums across the following modules: Fundamentals of Pain Science and Practice (PHTY20290), and Digital Transformation in Physiotherapy. Communication On the 28th of November the project promoted itself at the annual conference of the DGNR (Deutsche Gesellschaft fur Neurorehabilitation) in Düsseldorf. On December 12 we were chosen to present the project at the Dutch federation of Rehab Institutes (RevalidatieNL). The project was then chosen as a highlighted project to be scaled up all around the Netherlands. The concept of ScaleUp4Rehab, and HEROES of the Sea, was presented at the Dutch-Flemish Movement Analysis conference (SMALLL; Ghent, Oct 2025) and the Dutch Congress of Rehabilitation Medicine. Two abstracts on a study involving Parkinson's Disease patients have been submitted for upcoming conferences (Neurorehabilitation and Neural Repair, and the International Society for Posture and Gait Research).

VR4REHAB - Interreg 2014-2022 NWE (EN) | The majority of the consortium (SMK, ULille, UCC, In4Care, HV) was involved in VR4REHAB aimed at developing VR interventions for different types of rehabilitation patients. The knowledge gained has led to the initiation of this project. In SCALE-UP4REHAB we want to build on the project results and the network which was built to scale up of work done in previous applications (EN), ‘Technology in undergraduate Physiotherapy Education’ - the Erasmus+ Programme Key Action 220 Cooperation Partnerships in higher education [2022-1-IE02-KA220-HED-000087620]. (EN) | UCC and UCD are partners in this project. The work UCC and UCD undertake in the ‘Technology in undergraduate Physiotherapy Education’ project will complement and inform the work in activity 3.4 the XR intervention in the physiotherapy curriculum capacity building component of the ScaleUp4Rehab project (EN), NEST (AAL) (EN) | SMK is partner in the AAL project NEST. By leveraging widely distributed technologies among the population, such as smartphones, smartwatches, PCs, and laptops, NEST provides affordable and scalable science-based solutions for tele-rehabilitation. NEST will improve long-term recovery while allowing hospitals to reduce the length of outpatient treatment and its associated costs of infrastructure and staff while maximizing the treatment capacity of therapists. The network and knowledge acquired in this project will be used for developing the Virtual Rehabilitation 'Clinic'. (EN), Multitouch (MSCA-ITN) (EN) | Multitouch aims at providing high level training to a new generation of Early Stage Researchers (ESR) in the multidisciplinary field of haptics. ULille is partners in this project. (EN), IT4Anxiety (Interreg NWE) (EN) | IT4Anxiety (Interreg NWE) focuses on the creation and implementation of digital solutions, with the objective of reducing the anxiety of patients suffering from neurodegenerative diseases or post-traumatic stress disorder. There are close contact between the project partners and the IT4Anxiety consortium about the developed interventions and the ways to implement them and have them reimbursed. (EN), HEROES: Home-based ExeRgaming fOr Enhancing resistance to falls after Stroke (IMDI - Dutch grant) (EN) | In this project, the HEROES system will be developed, a novel exergaming system for home use that allows people with stroke to increase their resistance to balance perturbations. SMK and RUMC are partners in this project. (EN), FaceRehab (AAL) (EN) | FaceRehab develops a solution for supporting facial rehabilitation exercises at home using technology as a mean to improve digital transformation of the health and care for people affected with Facial Paralysis. REHAZENTER is involved as end-user in this project (EN), COGNIVITRA (AAL) (EN) | COGNIVITRA develops an ICT-based solution for cognitive vitality training at home. COGNIVITRA will integrate components for supporting cognitive and physical exercises – dual-task training – (web-based platform and movement sensors), which will facilitate the interface and communication between patients and care providers. REHAZENTER is end-user in this project. (EN)
