Demographic trends spur healthcare demand. inadequately met by healthcare institutions. One solution is more efficient services such as telehealth provision. PPs have joined forces to overcome problems/barriers linked to lack of support for businesses to market new. smart telehealth products. A bad connection of smart ICT products with quickly changing demands in the health market is also a challenge. In response. RECAP uses a growing health market as a business opportunity. to stimulate market uptake/use of telehealth in diagnosis. treatment & monitoring of patients adding value for business. patients & healthcare institutions. The telehealth sector in PP regions employs cc.110000 people with 145 SMEs & some bigger companies. Core sectors include technology. product & service providers. e.g. Philips Research. Philips Healthcare. Biomarker. Medtronic. IEM GmbH Stolberg (telemonitoring devices). Telehealth Solutions. Motiva. Simac (care solutions). Verklizan (services). For efficient enhancement of telehealth facilities. PPs have identified 3 crucial aspects. based on existing regional telehealth models: 1. intelligent devices. 2. intelligent organisation of care. 3. intelligent systems supporting decision-making. Analysis/testing linked to these aspects will be done by 3 regional hubs. each having specific expertise. focusing on Chronic Heart Failure. The hubs are building blocks to the transnat. Health Innovation Portal. The upscaling of regional care portals to a transnat. level is triggered by PPs’ common drive for optimalising delivery (critical mass) & keeping a high quality of care services. which provides health & other benefits for patients. e.g. reduced casualties. hospitalisations. increased convenience. safety. mental wellbeing. avoided/postponed medical surgery. improved ICT communication knowledge & life style changes. Increased awareness and demand from patients for telehealth services will in turn trigger business uptake.

Achievements (EN):

A lot of activities are still ongoing in the RECAP project, like the interviews with patients, GPs and hospitals in order to come up with a transnational comparative analysis of HF care and the analysis and piloting of home monitoring devices. However, some RECAP "spin-offs" are already worthwile mentioning here. The Early Warning Score (EWS) study from the Catharina Hospital Eindhoven turned out to be key in detecting derailed patients. Therefore the study is expanded from a ward environoment to home situations as well. Based on this RECAP success the Catharina Hospital obtained a grant to explore Early Early Warning Scores (EEWS) in a home situation. Currently all patients are treated the same. However, after a short interview professionals will get a better picture of the older patient and their home situation and activities, which helps professionals to better prepare the patient and make them able to take specific measures to undergo the surgery in the best possible way. Because of the RECAP insights also other studies of the Catharina Hospital are now being expanded to a home situation, like e.g. Thoracic Electrical Impedance measurements to measure changes in the amount of moisture inthrathoracaal (by electrodes in shirt) for detecting impending cardiac decompensation. Another promising method to optimise home medication is the examination of the amount of blood in the lungs, which can now only be done in the hospital, but can be combined with impedance measurements to see how the heart responds to this fairly acute change to give a much better picture of the cardiac reserve to optimise home medication.
