Description (EN): More and more citizens are suffering from hearing and balance problems, which are disorders related to the inner ear. For some, both, as it is the same sensory organ that is affected, and for others, only one of the parts. In isolation, it is estimated that one in six Danes has a hearing loss requiring treatment, while half of the 80-year-olds have or have had debilitating dizziness. Common to the two inner ear-related disorders is that the disorders are both hidden disabilities, that the conditions can rarely be cured with medication or surgery and that the disorders often have severe consequences for both the affected and their relatives.
Loss of sense of hearing and balance hampers daily activities and results in reduced functional capacity, risk of falls, as well as social isolation and loss of earning capacity. Consequences can be depression and dementia. The socio-economic impact is significant. There is a large group of vulnerable and resource-intensive citizens, where access to healthcare can be done through several routes. Some are referred to audiologists, ear, nose, and throat doctors or neurologists, while others come into emergency care. Citizens are often throwing balls between different medical specialties, as it can be difficult to determine the cause of dizziness, for example. This means that diagnosis often becomes long-lasting and thus it can take a long time before the right treatment is initiated. Through the ÖKS bridge-builder project "Menière" (disease that causes seizures with hearing loss and dizziness), the project has experienced a potential for using new technology and telemedicine solutions, targeting this vulnerable group with hearing loss and dizziness. The purpose of Öre-health is to build on these experiences so that the technology is developed and disseminated to disorders related to the inner ear. In this way, the project can both diagnose and treat citizens who suffer from both, and at the same time make use of the technological experience in order to develop and implement technological solutions for citizens who either only have hearing loss or only suffer from dizziness. The project expands the established collaboration across Øresund to solve common challenges in collaboration with the industry for the benefit of a larger group of citizens who have inner ear-related disorders. At the same time, the project can gain more knowledge about the extent to which the two disorders are related to each other by screening patients and by generally raising awareness and initiating a well-established collaboration across the two medical specialties throughout the programme region. The project actors will create a thorough evidence base and will test and develop technological solutions, and then disseminate these to the rest of the programme region for better and faster investigation, treatment and rehabilitation in collaboration with the industry.
This will be done by starting from the two conditions in relation to developing and testing technologies for both investigation, treatment and rehabilitation as part of the treatment. At the same time, the project will investigate the overlap between the two disorders both purely diagnostic, but also in relation to how it is experienced to live with a hidden disability, how the new technologies are experienced and used by the citizen, and not least what impact it has on the citizen's quality of life. In addition to the medical aspects, the project will get a much better insight into what it is like to be examined and treated when a citizen has a hidden disability in the form of a disorder related to the inner ear. The project will develop and use several new technologies, including newly developed apps, technological equipment for home monitoring and digital visitation, and it is therefore important to look at the advantages and disadvantages of the technologies from a health economic perspective. This is both in terms of health expenditure, consumption of health services and from a patient's perspective.
Read more Description (DA): Flere og flere borgere bliver ramt af problemer med hørelse og balance, som er lidelser relateret til det indre øre. For nogles vedkommende begge dele, da det er samme sanseorgan, der rammes og for andres vedkommende kun den ene af delene. Isoleret set så skønnes, at hver sjette dansker har et behandlingskrævende høretab, mens halvdelen af de 80-årige har eller har haft invaliderende svimmelhed. Fælles for de to indre øre-relaterede lidelser er, at lidelserne begge er skjulte handicap, at tilstandene sjældent kan helbredes med medicin eller kirurgi og at lidelserne ofte har svære konsekvenser for både de ramte og deres pårørende. Tab af høre- og balancesansen hæmmer de daglige aktiviteter og medfører nedsat funktionsevne, risiko for fald samt social isolation og tab af erhvervsevne. Følgevirkninger kan være depression og demens. De samfundsøkonomiske konsekvenser er betydelige. Der er tale om en stor gruppe udsatte og ressourcekrævende borgere, hvor indgangen til sundhedsvæsenet kan ske ad flere veje. Nogle bliver henvist til audiologer, øre-, næse-, og halslæger eller neurologer, mens andre kommer ind på akutmodtagelsen. Borgerne bliver ofte kastebold mellem forskellige lægefaglige specialer, da det fx kan være vanskeligt at afgøre årsagen til svimmelheden. Det betyder, at diagnosticering ofte bliver langvarig og derved kan der gå lang tid før den rette behandling bliver iværksat. Igennem ÖKS-brobyggerprojektet "Menière" (sygdom der giver anfald med hørenedsættelse og svimmelhed) har projektet erfaret et potentiale for at anvende ny teknologi og telemedicinske løsninger, målrettet denne sårbare gruppe med høretab og svimmelhed. Formålet med Öre- sund er at bygge videre på disse erfaringer, så teknologien udvikles og udbredes til lidelser relateret til det indre øre. På den måde kan projektet både diagnosticere og behandle borgere, der lider af begge dele, og samtidig gøre brug af de teknologiske erfaringer med henblik på at udvikle og implementere teknologiske løsninger til borgere, der enten kun har høretab eller kun lider af svimmelhed. Projektet udvider det etablerede samarbejde på tværs af Øresund for at løse fælles udfordringer i samarbejde med industrien til gavn for en større gruppe borgere, som har indre øre-relaterede lidelser. Samtidig kan projektet, opnå mere viden om, i hvilket omfang de to lidelser er relateret til hinanden ved at screene patienter og ved i det hele taget at skærpe opmærksomheden og initiere et veletableret samarbejde på tværs af de to lægefaglige specialer i hele programregionen. Projektaktørerne vil skabe et gennemarbejdet evidensgrundlag og vil teste og udvikle teknologiske løsninger, for herefter at udbrede disse til resten af programregionen til bedre og hurtigere udredning, behandling og rehabilitering i samarbejde med industrien. Det vil gøres ved at tage udgangspunkt i de to lidelser i forhold til at udvikle og teste teknologier til både udredning, behandling og rehabilitering som en del af behandlingen. Samtidig vil projektet undersøge overlappet mellem de to lidelser både rent diagnostisk, men også i forhold til hvordan det opleves at leve med et skjult handicap, hvordan de nye teknologier opleves og anvendes af borgeren, og ikke mindst hvilken betydning det har for borgerens livskvalitet. Udover de lægefaglige aspekter vil projektet på tværs af specialer og på tværs af lande få et langt bedre indblik i, hvordan det er at blive udredt og behandlet, når man som borger har et skjult handicap i form af en lidelse relateret til det indre øre. Projektet kommer til at udvikle og anvende flere nye teknologier, bl.a. nyudviklede apps, teknologisk udstyr til hjemmemonitorering og digital visitation, og det er derfor væsentligt at kigge på teknologiernes fordele og ulemper i et sundhedsøkonomisk perspektiv. Dette både i forhold til sundhedsudgifter, forbrug af sundhedsydelser og fra et patientperspektiv.
