In 2010 the World Health Organisation launched the initiative Age-friendly World. The vision is to reshape all towns and regions into an exceptional (!) living space in which to grow old. The project Age-friendly Region will contribute towards bringing this vision closer to the Hungary-Austria border region. Old age and care is a growing challenge for our societies. Previous ways of coping, such as a stronger commitment by relatives or the increased development of residential homes, are hardly suitable any more for improving the situation of older people who are in need of care. For this reason, the Age-friendly Region project will develop new approaches so that everyone in the region (and their relatives) can remain in their familiar surroundings, with a high quality of life and as independently as possible, even in old age. The cross-border cooperation allows the knowledge and experiences of both countries to be used and for a common model to be developed that builds on this. Six case managers will support 200 people with new systematic approaches for developing individual accommodation concepts as part of pilot projects, which are coordinated as much as possible with particular living and accommodation arrangements, and which are used in an optimal way with existing resources. The joint academic partners will analyse the experiences and effects of these new services in accordance with academic standards and open up the possibility of learning systematically from the differences between the two countries. For a long time, the strategies of the two countries on health have sought to improve support for older people in the organisation of everyday life and care at home and the better regional coordination of related services. Age-friendly Region will be an important step in this direction by the region and for the first time it will establish bilateral cooperation between AT and HU in this area.

2010-ben a WHO megalapította az Age-friendly World kezdeményezést, amely szerint minden városnak és régiónak olyan nagyszerű élettérré kell válnia, ahol érdemes megöregedni. Az Age-friendly Region hatékonyan elősegíti, hogy Magyarország-Ausztria határtérsége ehhez az elképzeléshez közelebb kerüljön. Az időskor és ezzel összefüggésben a növekvő ápolás, gondozási szükségletek egyre nagyobb kihívás társadalmunk számára. A meglévő megoldási alternatívák erősségei – mint a hozzátartozók odaadó támogatása vagy a gondoskodást biztosító kapacitások folyamatos emelése – már nem képesek tovább fokozni az idősek életminőségét. Az Age-friendlyRegion projekt ezért új ötleteket, javaslatokat dolgoz ki, hogy a régióban lakó idősek és hozzátartozóik magas színvonalú életkörülmények között, a lehető legnagyobb autonómiával megszokott környezetükben maradhassanak. A határon átnyúló együttműködés lehetővé teszi, hogy a két ország tudását és tapasztalatait alapul véve közös modellt dolgozzunk ki. Több, mint 200 fős kísérleti projektek keretében 6 esetmenedzser fog segíteni új módszertani ötletekkel abban, hogy kialakítsák a helyi szükségleteken alapuló ellátási koncepciókat, melyek az egyén élet-, és lakóhelyzetéhez igazodnak és a meglévő források segítségével a szolgáltatások hatékonyabb kihasználását jelentik. A közösen kiválasztottkutató-elemzőpartner az új szolgáltatások hatását tudományos standardok szerint fogja kiértékelni, a külön kiemelve a két ország különbözőségeiből adódó tanulási lehetőségeket. A két ország szociális és egészségügyi stratégiái már jó ideje arra irányulnak, hogy az idős emberek a saját otthonukban szükségleteiknek megfelelő, a szolgáltatást biztosítók által hatékonyabban összehangolt támogatást kapjanak regionális szinten. Az Age-friendly Region egy fontos mérföldkő lesz ennek a célnak az elérésében és elsőként hoz létre bilaterális együttműködést Ausztria és Magyarország között ezen a szakterületen.

2010 hat die Weltgesundheitsorganisation die Initiative Age-friendly World gegründet. Vision ist die Umgestaltung aller Städte und Regionen zu einem großartigen (!) Lebensraum, um alt zu werden. Das Projekt Age-friendly Region wird dazu beitragen, die Grenzregion Ungarn - Österreich dieser Vision näher zu bringen. Alter und Pflege ist eine wachsende Herausforderung für unsere Gesellschaften. Bisherige Bewältigungsformen wie ein verstärktes Engagement von Angehörigen oder ein stärkerer Ausbau von Pflegeheimen sind kaum mehr zur Verbesserung der Situation ällterer pflegebedürftiger Menschen geeignet. Das Projekt Age-friendly Region wird deshalb neue Ansätze entwickeln, damit alle Menschen der Region (und ihre Angehörigen) auch im Alter in hoher Lebensqualität und größtmöglicher Autonomie in ihrer gewohnten Umgebung verbleiben können. Die grenzüberschreitende Zusammenarbeit ermöglicht es, Wissen und Erfahrungen beider Länder zu nutzen und darauf aufbauend ein gemeinsames Modell zu erarbeiten. 6 Case-ManagerInnen werden im Rahmen von Pilotprojekten über 200 Personen mit neuen methodischen Ansätzen dabei unterstützen, individuelle Versorgungskonzepte zu erstellen, die optimal auf die jeweiligen Lebens- und Wohnsituationen abgestimmt sind und durch die vorhandene Ressourcen bestmöglich genutzt werden. Der gemeinsame wissenschaftliche Partner wird die Erfahrungen und Wirkungen dieser neuen Dienstleistungen nach wissenschaftlichen Standards auswerten und die Möglichkeit eröffnen, von den Unterschiedlichkeiten beider Länder systematisch zu lernen. Die Gesundheitsstrategien beider Länder streben seit längerem eine bessere Unterstützung älterer Menschen bei der Organisation von Alltag und Pflege zu Hause und eine bessere regionale Abstimmung der diesbezüglichen Dienstleistungen an. Age-friendly Region wird ein wichtiger Schritt der Region in diese Richtung sein und erstmals eine bilaterale Zusammenarbeit zwischen AT und HU in diesem Bereich etablieren.

Achievements (EN):

The overarching goal of this bilateral project was the close cooperation of the border regions Austria and Hungary on work with the elderly and any related challenges, such as population ageing and skill distribution of professionals in care management. The Age-friendly Region project aims to improve the quality of life of affected older adults and their relatives in these regions. The approach is holistic and endeavours to achieve lasting improvement using the Case and Care Management (CCM) Model. The resulting collective knowledge is available as know-how and represents trans-national learning on both sides. The project results divide into two main areas: first, the findings of scientific evaluation reports based on the comparative quality of life surveys of Case Management (CM) clients (before and after availing of CM services), as well as the interviews of actors in the health, care and social sectors. Second, the findings provide practical experience based on the implementation of regional CCM models as a CM service in both project regions. As a scientific partner, the European Centre provided professional support throughout the project, for example, with data collection and evaluation, yielding methodological information on the development of CM clients. Thus, we can make a central statement. Compared to people that have not received AFR CCM, AFR customers’ subjective health and overall quality of life have undergone a positive change in both countries. A significant function of CM was to identify any rural gaps in care infrastructure to stimulate potential improvements, for example, by utilising information for system-relevant care management. Healthcare actors of both Austria and Hungary also reported positive trends in the reach and strengthening of networking, which is a result of the cooperative CM service. Regional lifestyle differences in conjunction with the service landscape and any related requirements are essential considerations for a successful CM service. The practical empirical values from the CM realisation were evident towards the end of the project. They were recorded using a guide to implementing CCM in these regions. The guide provides direction while representing a know-how transfer of practical client-focused care (for professionals in Austria and Hungary). The guide highlights that a suitable model and strategy is necessary to provide a high-quality service for older people in these regions. This permits a holistic approach to service provision and the opportunity to create added value for the target group. The cooperation of the cross-border project partners nurtured new knowledge in various areas, which had a tremendously positive impact on the course of the project. Each partner benefited from close cooperation and various on-site training courses, information events, planning meetings, and specialist conferences. The knowledge gained strengthened the skills of professionals. It also served as a motivator for sustainable work with the elderly, their relatives and networks. The shared project goals to ‘improve the quality of life of affected older adults and their relatives’ and create awareness for the ability to ‘live at home as a senior citizen’ of this bilateral partnership between Austria and Hungary created a shared connection on existing regional development structures in the health, care and social sectors.

A kétoldalú projekt elsődleges célja az Ausztria/Magyarország határrégió intenzív együttműködése volt az idősekkel kapcsolatos munka és az ezzel összefüggő kihívások vonatkozásában, mint pl. a lakosság egyre emelkedő átlagéletkora és a szakértelemmel bíró munkaerő eloszlása az ápolás-menedzsmentben. Az Age-fiendly Region projekt feladataként tűzte ki azt, hogy teljes körű CCM-modell segítségével tartósan javítja a régióban élő érintett idős emberek és azok hozzátartozóinak életminőségét. Az ebből származó közös tudás most, mint tudásbázis áll rendelkezésre és résztvevőinek határon átnyúló tanulását reprezentálja. A projekt eredményei két lényegi részből tevődnek össze, egyrészt a tudományos kiértékelési beszámolók ismereteiből–melyek a CM ügyfeleinek életminőségével kapcsolatos összehasonlító felméréseken (a CM-szolgálat igénybevétele előtt és után) valamint az egészségügyi, ápolási és szociális szektorban (Care-Management) dolgozó résztvevők megkérdezésén alapulnak. Másrészt pedig mindkét projektrégióban a regionális CCM-modell, mint CM-szolgáltatás, megvalósítása során szerzett gyakorlati tapasztalatokból. A projektet az European Centre tudományos partner a teljes futamidő alatt figyelemmel kísérte és támogatta, úgy az adatszerzés, mint a kiértékelés terén, és ezáltal módszertanilag helytálló információkat szolgáltatott a CM ügyfeleinek fejlődéséről. Ennek megfelelően kijelenthetjük, hogy az AFR ügyfeleinek szubjektíven észlelt, egészségügyi vonatkozású és általános életminősége vonatkozásában összességében mindkét országban pozitív változás volt tapasztalható, azon személyekhez viszonyítva, akik nem részesültek az AFR Case & Care Management-jében. A Case Management fontos feladata nem utolsó sorban az volt, hogy felfedje az ápolási szakterület infrastruktúrájában meglévő hiányosságokat vidéken és ezáltal kezdeményezze ennek lehetséges javulását, javítását (azáltal hogy az információkat bejuttatja a rendszer szempontjából fontos Caremanagement-be). Mindkét ország egészségügyi szektorból érkező résztvevői ugyanakkor pozitív tendenciáról számolnak be a hálózat hatótávolsága és erősödése vonatkozásában, ami a kooperatív CM-szolgálatnak köszönhető. A sikeres Case Management-szolgálat lényeges részeként kell megemlíteni az emberek életfeltételei között meglévő regionális különbségeket az integrált, átfogó ellátás és az ezzel kapcsolatos igények vonatkozásában. A Case Management megvalósítása során szerzett szakmai tapasztalatok a projekt végső fázisban váltak jól láthatóvá és egy Útmutató formájában rögzítésre kerültek a CCM régiókban történő megvalósítása érdekében. Az Útmutató egyfajta segédlet és egyben a gyakorlatias, ügyfelekkel kapcsolatos munka tudásbázisa (a magyar és osztrák szakemberek számára). Az Útmutató kihangsúlyozza, hogy szükség van egy alkalmas modellre és egy stratégiára ahhoz, hogy a régióban élő idős embereknek minőségi szolgáltatást lehessen nyújtani. Ez lehetővé teszi, hogy minden aspektust figyelembe vegyünk a munka során valamint hogy a célcsoport számára a lehető legtöbb hasznot. A határközeli projektpartnerek együttműködésének eredményeként felismerhetővé váltak a két ország ápolási szakterületének hasonló ill. eltérő elemei, melyek rendkívül pozitívan hatottak az együttműködésre a projekt ideje alatt. A partnerek sokat profitáltak az intenzív együttműködésből és a számos helyszínen zajló továbbképzésekből, tréningekből és információs rendezvényekből, valamint a tervezett találkozókból és szakmai konferenciákból. A megszerzett ismeretek révén bővült a szakemberek kompetenciája és ezzel együtt minden résztvevő számára rendkívüli motivációt jelentett az idősödő lakossággal, családtagjaikkal és környezetükkel történő tartós munka megteremtéséhez. A közös célokkal – „az érintett személyek életminőségének javítása“ valamint az „időskori otthonélés“ tudatának megteremtése - az Ausztria és Magyarország közti bilaterális együttműködésnek sikerült összeköttetést teremteni a regionális fejlesztés meglévő struktúráiban az egészségügyi, ápolási és szociális szektorban.

Das übergeordnete Ziel dieser bilateralen Projektarbeit war das intensive Zusammenwirken der Grenzregionen Österreich/Ungarn in Hinblick auf die Altenarbeit und den damit verbundenen Herausforderungen wie etwa ein steigendes Durchschnittsalter der Bevölkerung und die Kompetenz-Verteilung der Fachkräfte im Pflege-Management. Das Projekt Age-fiendly region hat es sich zur Aufgabe gemacht, die Lebensqualität der betroffenen älteren Menschen und deren Angehörigen in den Regionen, durch einen ganzheitlichen Ansatz im Case- und Caremanagement-Modell (CCM), nachhaltig zu verbessern. Das daraus entstandene gemeinsame Wissen steht als Know-How zur Verfügung und repräsentiert ein grenzüberschreitendes Lernen der beteiligten Akteure. Die erarbeiteten Projektergebnisse gliedern sich in zwei wesentliche Bereiche, zum einen die Erkenntnisse der wissenschaftlichen Evaluationsberichte – basierend auf den vergleichenden Lebensqualitäts-Erhebungen von KlientInnen des Case Management (vor und nach der Inanspruchnahme des CM Dienstes) und den AkteurInnen-Befragungen aus den Bereichen Gesundheit, Pflege und Soziales (dem Care-Management). Zum anderen liefern die Ergebnisse praktische Erfahrungen anhand der Implementierung der regionalen CCM-Modelle als CM-Dienstleistung in den beiden Projektregionen. Das Projekt wurde über die gesamte Laufzeit hinweg durch den wissenschaftlichen Partner European Centre professionell in der Datenerhebung, wie auch Evaluierung, begleitet und lieferte dadurch methodisch-fundierte Informationen über die Entwicklung der Klientinnen des Case Managements. Demnach kann die zentrale Aussage getroffen werden, dass die subjektiv wahrgenommene, gesundheitsbezogene und allgemeine Lebensqualität der AFR Klientinnen und Klienten insgesamt in beiden Ländern eine positive Veränderung erfahren haben, dies im Vergleich zu Personen, die kein AFR Case & Care Management erhielten. Eine wesentliche Funktion des Case Managements bestand nicht zuletzt darin, bestehende Lücken der Pflege-Infrastruktur im ländlichen Raum zu identifizieren und dadurch eine mögliche Verbesserung anzuregen (durch das Einspielen von Informationen in das systemrelevante Caremanagement). Die AkteurInnen aus den Gesundheitsbereichen beider Länder sprechen darüber hinaus von einer positiven Tendenz, bezogen auf Reichweite und Verstärkung der Vernetzung, welche sich durch den kooperativen CM Dienst entwickelt hat. Als wesentliche Betrachtung für einen erfolgreichen Case Management Dienst, sind die regionalen Unterschiede in den Lebensbedingungen der Menschen in Verbindung mit der Versorgungslandschaft und damit verbundenen Ansprüche zu berücksichtigen. Die praktischen Erfahrungswerte aus der Umsetzung des Case Managements wurden in der Endphase des Projektes reflektiert und mittels eines Leitfadens zur Implementierung von Case und Care Management in Regionen festgehalten. Der Leitfaden dient als Orientierungshilfe und ist gleichzeitig ein Know-How-Transfer der praktischen klientenInnenbezogenen Arbeit (für das Fachpersonal in HU und AUT). Der Leitfaden hebt hervor, dass es ein geeignetes Modell und eine Strategie braucht, um einen qualitativ hochwertigen Dienst für ältere Menschen in Regionen anzubieten. Dies ermöglicht einen ganzheitlichen Ansatz in der Arbeit, sowie die Möglichkeit einen Mehrwert für die Zielgruppe zu generieren. Die Zusammenarbeit der grenznahen Projektpartner brachte Erkenntnisse in verbindenden und unterscheidenden Elementen, welche sich äußerst positiv auf die Zusammenarbeit im Projektverlauf auswirkten. Die Partner profitierten von der intensiven Kooperation und den zahlreichen vor Ort stattfinden Weiterbildungen, Trainings und Informationsveranstaltungen, sowie planerischen Treffen und Fach-Konferenzen. Die erworbenen Kenntnisse stärkten die Kompetenzen der Fachkräfte und waren gleichzeitig Motivator für die Schaffung nachhaltiger Arbeit mit der alternden Bevölkerung und deren Familienangehörigen und deren Netzwerke. Durch die gemeinsamen Projektziele – die „Lebensqualität der betroffenen Personen zu verbessern“ und das Bewusstsein für „im Alter zu Hause leben“ zu schaffen, schaffte diese bilaterale Partnerschaft zwischen Ungarn und Österreich eine verbindende Wirkungen auf die jeweils vorherrschenden Strukturen der Regionalentwicklung in den Bereichen Gesundheit, Pflege und Soziales.
